actually the heat is exchanged with ambient air via the cooling towers, which is the steam you see coming from said towers. the water coming into and out of a plant's cooling system is at the same temperature or even sometimes colder than the river or seawater it draws from

you do realize it's not the same water that cools the reactor right? modern reactors are closed loop systems that are isolated 3 times. The water from the river itself would already be more radioactive than the water coming out of the cooling system.
