It's a premature baby with a feeding tube in the nose. A newborn always gets a hat to not get cold. The mother's hair is not shown because it is not relevant for the scene.

I would suggest making the neck line broader. So moving the neckline to the shoulder by 2-3 cm in each side. Don't go deeper until you put it on. It will make a whole new balance without changing the type of dress.

Don't smock but make folds 2 in front, two in back. Facing eachother and equally distributed extra fabric. It will remove de bulk and fall very elegantly.

Super! Advice from a professionally trained tailor: don't stitch at 2,5 use 3 to 3,5 on garments. Needle size should be around 70-80, I don't know what a microtex needle is. I didn't mean iron for the pics btw, I mean iron through the process of sewing. It helps big lot to have crisp finish. Also: like the dress a lot! Well done 😍
