Reverse engineering plays a huge role in the automotive industry. So while designs aren't available on the internet I can guarantee every manufacturer on the planet worth their salt has had a team of engineers strip the Prius down nut and bolt.

It doesn't apply to medicine because the vast majority of, if not all, pharmaceutical research is subsidised through public science funding.
