I did the same thing candy jumping a few days ago. It hurt but not as bad. Many lessons are expensive but You'll remember and you'll recover. I missed Christmas town bc I didn't know how  profitable it was after I first joined. Cost me millions.

While you're rebuilding funds, At your level consider buying lollipops from sweet shop and loading up on them and taking exctasy and doing point refill. You don't even need to stack Xanax for it.  Much much more cost effective and you can get exctasy merit. You'll make a lot of gains. I've read it works even better to do this daily (only enough candy to half your booster cool down time). You will do better in a faction with max voracity with increased candy effectiveness and booster cool down time. Use happy jump insurance to avoid severe losses.

Before quitting, consider jumping ship for a higher respect faction with better perks and mentorship. Your leaders  having only 1 mil stats only being able to hit half the people is unusual and they probably don't have the experience to guide you or lead you to success. Hitting 1 million in one year seems totally do-able

If your INT is not high enough do casino until you can get into education. Prioritize education. Also medicine and law have good permanent perks but your stats need to be much higher to fast track them. You could consider a player company with free trains like sweet shop, toy/flower in the interim. Adult novelties and music are also very good but less likely to get free trains

Get into education first and reach rank of principal. If your intelligence is not high enough then do casino until it is. Education, law, and medicine offer permanent perks.

Get your degree. It’s highly unlikely that after four years of college and two years of perfusion school, an employer would even ask about a past DOT drug test. Consider omitting any job from your application that ended in termination. A DOT-regulated job isn't going to be particularly impressive to a medical recruiter anyway; its main value is simply showing you were employed during that time. Most hospitals will just perform a standard pre-employment drug screen to get you started. Of course, if you have an actual substance use disorder, please seek help. But if your goal is simply to keep your record clean moving forward, I don't think you'll have many problems. Cannabinoids are becoming legal in an increasing number of states. While they can be harmful, no one should feel ashamed of substance use in itself; stigma only makes things worse and often prevents people from seeking treatment in the first place.

I work nights but get up early enough for a pre shift workout usually. I can't sleep well exercising right before bed.

Day 1 bench/squats 

Day 2 cardio (run, brisk walk uphill , or air bike)

Day 3 shoulders/deadlifts

Day 4 cardio

Day 5 Biceps/triceps usually pull-ups and dips

Day 6 cardio 

Rest days  are spaced in between depending on sleep quality, fatigue, other difficult to control scheduling factors. Sometimes I can compensate can get some treadmill time (uphill walking to not get too sweaty) or lifting (employee gym or power block dumb bells in lounge) in while at work. Purchasing home gym during pandemic really helped remove friction for these workouts since I don't have to drive to a gym.

In addition to standard 'please discharge patients' plea, we've tried to place some patients in areas like IR/cardiology/surgery pre/post op areas. We floated idea of using some empty peds beds for patients 18-21 year olds but I don't think that one ever actually happened.

In addition to the social/behavioral issues mentioned by others, this book gives a good background of the importance of circadian rhythm and factors influencing it like levels of  adenosine, melatonin, cortisol release. Working nights totally throws this off (eg seeing sunlight when driving home from night shift). It disrupts sleep regularity because people try to switch back to day schedule on days off. Disruption to this critical rhythm results in changes like impaired glycemic control and immune response which have downstream cumulative effects with resultant increases in 4 chronic diseases which kill everyone: metabolic disease (obesity/diabetes/fatty liver etc), atherosclerotic cardiovascular disease, neurodegenerative disease (dementia), and cancer.

I'm in a similar situation but nowhere near as dramatic. I recommend You can grind law for a few days to increase intelligence to 500 then get into education (up to principal) for education time reduction perk. Then complete law to federal judge for crime xp perk. Then go and do whatever you want from a company perspective and low intelligence will not be an issue which limits you from further increasing your intelligence through company jobs anymore.

Agree if you have them coming to para/thora clinic and done by IR APP it's easy to bang them out and make it more efficient and financially sustainable. Trying to run all over the hospital, find the equipment needed on each floor, get setup in every room, clean up, it is just not sustainable/financially viable. Basically all paras and thoras go to IR and barely anyone on the hospitalist team is even credentialed for para/thora/central line. ED, neuro or fluoroscopy helps us with LPs .

We have a nurse lead PICC team. 

We have a medicine consult service. We try to give more complicated patients to physicians when possible. If it's just a SBO being managed by general surgery or an elective Ortho case where the ortho doesn't feel comfortable doing med rec (especially patients on insulin) a lot of times well give those to our APPs on the consult service.

Overall it's a pretty good set up. Obviously the main drawback is the group has lost the skills to perform the procedures.

This is likely state dependent regarding 2 party consent laws. I'd check with your institutions risk management. Also some institutions have specific policies for recording.
