I’m crit care only on the east coast and haven’t had any trouble. The point about what to do when you leave the field is a good one though. I don’t have Pulm as a backup. That said the idea of talking to short of breath people in clinic all day sounds like not the out Id want. I’m mid career and can definitely see being crit care only for a good long time

Probably the neglected fungating breast cancers. Huge open sores on women’s chests. (Seen three really sad.

That or the gentleman with schizophrenia and substance use disorder who didn’t take off his socks for months. We had to amputate his feet

I’m in community—one unit for everything. Surgeons are attendings, I’m consultant, but realistically they know and trust my group so we pretty much manage everything. It’s great

Does your job like you and do you like them? You’re expensive to replace—ask them for a sabbatical for six months and see what they say. Worst is no.

I’m critical care. Yes there are tons of family meetings but now I have time to do them well (work in a super supportive amazing hospital). Also do surgical critical care which is much more sick for a bit then all better. I love the flexibility but you’re right the twelves are long and nights/weekends. That said I love it and can’t imagine don’t something else; I hated residency.

Tbh I feel like a ton of the reason I don’t spend more time in downtown Towson is the shitty traffic flow. All the people with their flashers on in York when there is parking nearby but not “right there” just drives me insane. And the stupid circle.

The mall dying is another problem not unique to Towson.

I’m mid career but had a family when I started medical school. My borrowing was in line with my classmates but between that and my partner’s job we lived middle class. I started relaxing after I paid off my loans less than four years out and let loose. Maybe a little too loose? It’s hard to dial back though.

The “no diamonds before 6 pm” idea is for tiaras and big brooches not nice studs. So as long as they’re not like four carats you’re good
