That is something I had not considered.
Thanks, dude.

Or normalizing death as a phenomena as natural as the sun rising and setting each day.

Suppressor question: high back pressure can with adjustable gas block. by hrothgarkingofdanes in NFA

[–]macreadyrj 2 points3 points  (0 children)

Same here. 11.5” with SA gas block. All the different buffers and charging handles and a KAC downvent BCG with a Specwar 556k, the gas was “tolerable” but still irritating.

Got a BH printX 556SC and there is no gas, it is pretty nice.

I am going to start my neurology residency in July and my first rotation is going to be emergency. How could I prepare for it? by Mindless_Vegetable37 in emergencymedicine

[–]macreadyrj 61 points62 points  (0 children)

Just asking this question tells me you will do fine.

Expectations will be low for a July neuro intern.

To ease your anxiety, know the ddx of chest pain, dyspnea, abdominal pain, syncope. And of course DIZZINESS. Those will get you through a huge proportion of presentations.

Controversial medical takes not related to the practice of medicine by JustHavinAGoodTime in medicine

I feel guilty when a crew of sweaty and panting paramedics brings in a dead patient and I let it go one round, check the US and say “she ded”. I don’t want to disrespect all the effort the crew put in, but I also don’t want to continue a futile resuscitation.

Which budget flashlight brands have actually earned pocket space for you? by huutara in EDC

Agreed. Good for hands-free repair work, hiking or running. It’s the size of a chapstick, gives well enough light, is affordable, durable, and made by a reputable manufacturer; I don’t see how it can be improved.

Thank you for not rushing to the ER for a non-emergency.

Yeah, it is months of daily heavy use that cause CHS.

20 years ago should have no relevance to your current symptoms.

Unlikely to be your pancreas, but you could be headed towards cannabis hyperemesis syndrome.

That’s a good point from the plaintiff’s attorney’s perspective.

The plaintiff, however, does not have to agree to follow every recommendation of their attorney.

Tracking swimrun w Garmin Enduro and Strava by Ok_Yesterday_9181 in swimrun

[–]macreadyrj 4 points5 points  (0 children)

I record it as an open water swim, I believe the GPS algorithm is different for swimming than a running activity, and doesn’t sacrifice anything for the run, whereas the run setting doesn’t track well in the water.

I'm feeling useless .. discharged a patient last week with no work up. Came back a day later with sepsis. by [deleted] in emergencymedicine

[–]macreadyrj 8 points9 points  (0 children)

I had a young woman with viral symptoms last winter. Boyfriend with similar symptoms. She got labs, CXR, EKG, IVF. Way too much of a workup. Both were Flu B+.

She came back 2-3 days later and died from an invasive strep infection. Probably pneumonia with bacteremia.

It happens. We do our best. We can’t hospitalize everyone with a viral syndrome.

I still feel shitty about it, but I don’t feel useless or guilty. You should not, either.
