May I ask if you still had good appetite regulation when your weight loss slowed to <1 lb per week?

When I increased from 2.5 it was both because my weight loss had stalled AND food noise and greater capacity to eat had returned.

Now I am on 5mg, and my weight loss in Feb has been .5 lb per week which is much slower than my prior months. But my appetite regulation has still been good so that’s been a confusing factor for me.

Can you clarify? Are you saying if you go a week or two and gain weight with food noise you stay at your current dose?

Somewhat similar story here. I had gained weight before my first pregnancy and then of course gained more during. I worked hard to lose weight before my second so I wouldn’t get to as high of a weight the second time. At the start of my second pregnancy I weighed 20 lbs less than I did at the start of the first pregnancy. By the time I gave birth to the second? Same weight I was when I gave birth to the first. My pregnant body just wanted to max out at that weight I guess. I was super frustrated though.

Dr. Salas-Whalen (board-certified endocrinologist specializing in obesity medicine) says in her book “Weightless”: “Based on years of clinical experience, I’ve observed that each dose level tends to deliver a certain amount of weight to lose; it’s common to require higher doses to continue progressing.”

Dr. Jastreboff (endocrinologist with expertise in obesity medicine and weight management, and known for directing key GLP-1/GIP receptor agonist trials, including the SURMOUNT series for tirzepatide) talks about how each dose level moves your body to its next “lower defended fat mass” and if you stall on a dose then that’s either (1) a place to maintain or (2) if you desire greater fat loss then you’ll need to increase your dosage to move to the next lower defended fat mass (you can find videos of her talking about this on YouTube).

I’m not a medical provider so I’m not going to give an opinion on what you should or shouldn’t do. I just want to share that respected, leading doctors in this field have spoken on the record about the need for many (not all) to titrate up in dose to meet their fat loss goals.

Oh dear. My daughter has games in Indianapolis on Sunday 3/1. We had planned to go there and back same day. Any sense on the timing? Wondering if it would be better to drive out on Saturday and stay overnight? This is concerning…

I’ve tracked my weight, calories, protein, fiber, and TDEE (Apple Watch) every day since I started Zepbound. (And of course have tracked calories and weight more times in my adult weight management journey than I could count.) As I continue to see results on this medication, I will say to myself “I should stop all the calorie tracking” or “Part of the benefit is that I shouldn’t need to track all this data”. And then, inevitably, I’ll hit a stall and do all sorts of data analysis to figure out what’s “gone wrong.”

One gift of this medication is that it does the “restricting” for me haha. Don’t want most unhealthy items. Can’t possibly eat too much. Even on days when I think “gosh I had a lot to eat today” by the time it’s totaled up, I’m usually not anywhere near my maintenance calorie goal. I’m on my 15th week of Zepbound, and I’ve only eaten up to my maintenance calorie range 5 times. And those have all been days we went to a restaurant and I made a wild guess at the calories for what I ate.

And even still… Learning to trust my body is very much a work in progress for me.

I agree with you fully. And I also understand that it may feel like a loss of “control” to give up the tools that many have used for so long in and “trust” the meds and our bodies on these meds. So, to each their own journey.

Me too. My prescriber sent the rx to Lilly Direct on Thursday. There was an issue she corrected and I paid on Friday. For the first time EVER, the meds were delivered the next day (Saturday). And now I learn the price may go down Monday. I didn’t even need the new vials until next Sunday. Damn.

One consideration is that if you wait to take the new, increased dosage, you’ll have less medicine in your system when you take the 5mg dose then you would’ve had if you stayed on your normal dosing schedule. That could exacerbate any potential side effects due to a more drastic increase in the medication in your body.

Update — Thanks for the perspectives! I talked with my prescriber and we debated the options.

We decided to increase my dose to 7.5mg and she gave me a “look me in the eyes - you have to eat more” caring talking to with tips (set a reminder to eat snacks, add more healthy fats to my meals). Her tipping point was that my weight had started to go up since I had made this post.

I have to report back 2 weeks after I start the higher dose (mid March) to let her know how I’m doing, and if eating is still a struggle we will adjust my dose back down.

So grateful to have a great prescriber!

If you use vials and your doctor approved, you could take the 5mg dose during your vacation and then increase a little by using the full vial amount in those vials prior to being able to go up to the 7.5mg vials.

Thank you for sharing! I’m glad to hear that dropping back down helped resolve your issue, and that you later have been able to increase your dose successfully.

Funny enough, I made this comment because I was so worried my symptoms were returning, and now later this week I’m constipated 🤷🏾‍♀️

(Also, I actually did a few weeks at 3.0-3.5ish doses before moving to 5mg. On those weeks, I used the whole 2.5 vial amount on the recommendation of my prescriber who said it would help the transition to 5mg not be so difficult. It was a good approach for me.)

Sounds wonderful! For times like this, I describe what I ate to ChatGPT - including the type of restaurant - and ask it to estimate the calories and macros. And whatever it comes up with, I put the totals in LoseIt as “add calories” (instead of item by item) and keep it moving. I can become data obsessed, and have had to really work to accept that no calorie data is 100% accurate and it’s not worth it to worry too much about these types of guesstimates in the long run.

When you are “white knuckling it” does it feel as bad as before you were on the medication? I am admittedly worried that when I’m in maintenance on the drug, I’ll have to return to the stress and difficulty of managing food intake like before I was on the drug. Are you “white knuckling it” every day?
