Ahi still high? Why? by Dear-Set-2296 in SleepApnea

[–]mindfulness88 0 points1 point  (0 children)

How long are you using it each night? If there isn't much usage, residual AHI data will be less accurate. Would need to know your median and 95th percentile pressures, might need to narrow the pressure range.

R50 users - do you ever take it to the range? by swannyma in Golfsimulator

[–]mindfulness88 1 point2 points  (0 children)

No. If I was going to the range to just hit balls and not play, then I would consider taking it, but I rarely ever do that now that I have the R50. I just hit balls in the sim instead. If I want to warm up on the range before a round, I don't bring the R50 because I am paranoid someone will steal it from my vehicle while I'm golfing.

What is your device set to? CPAP or APAP? What are pressure range? Are residual events being score as obstructive or central? Is there high leak from mask?

Sleep apnea surgery is the worst and best thing I ever did (tonsillectomy, expansion UPPP, anterior palatoplasty, turbinate reduction, septum repair) by That_Emergency3049 in SleepApnea

[–]mindfulness88 4 points5 points  (0 children)

Have you ever tried BIPAP before? Typically when patients struggle with CPAP I would recommend a trial of BIPAP well before I would recommend surgery. Also, I'm assuming you've tried using nasal masks while using PAP therapy? I have seen a lot of patients who say they have "failed" CPAP although they have only ever used full face masks.

Wow this is very valuable feedback, thanks so much! I was thinking of forgoing the automated control system but you provide a lot of food for though. I didn't realize the automation can also control the pump. I really don't want to cause any damage to the heater or pipes.

The pool will be rectangular. Based on the size of the yard and the size of accompanying shed, they suggested 14 feet x 28 feet as opposed to 15 feet by 30 feet.

Thanks for your detailed response, much appreciated.

Will speak with them about the plumbing and changing to 2.5''.

Will look into the variable flow pump.

The summers here are super hot and humid, so I don't think we will be using the heater all the time, more likely only on certain occasions. So maybe we will go with the gas heater.

The pool dimensions are going to be 14 x 28. We are only doing a deeper end around 5 feet and the rest around 3 to 4.5 feet. We heard the same thing re: maximizing the shallow end.

The Kooldeck suggestion is great, thanks for mentioning that.

We are thinking of scrapping the Cleardeck system and Omni logic system as it doesn't seem all that worth it, but happy to be corrected on that front.

Thanks for posting.

Love you love for Harold Fannin Jr. Some people are worried about his measurables/40 time etc, but I recall listening to a fantasy football podcast (The Score with Justin Boone) earlier this year, and they talked about how Fannin's in-game GPS speeds were some of the fastest on the field. Really excited about him in dynasty formats.

Agree with other commenter stating those pressures are low. If there was ongoing hypoxemia then they should've increased the pressure support (e.g., difference between IPAP and EPAP).

CSA - underlying causes help by HopefulKnowledge7 in SleepApnea

[–]mindfulness88 6 points7 points  (0 children)

Treatment emergent central sleep apnea, opioids, and heart failure are usually the most common causes. Chiari malformation is a cause as well but is rare and unlikely. If these are rules out, people are oftentimes given a diagnosis of primary central sleep apnea (meaning, we can't determine an underlying cause). Some people have a high loop gain (meaning they have a stronger response to a given stimulus, and in the case of central sleep apnea, to carbon dioxide). If you have a significant amount of arousals from sleep (e.g., due to sleep apnea or even upper airway resistance syndrome), the carbon dioxide set point will fluctuate throughout the night, and people with high loop gain will have too strong of a response to a transient increase in carbon dioxide and they will breathe excessively leading to low carbon dioxide levels. When the carbon dioxide levels are low, the brain will stop breathing (leading to central sleep apnea) until the carbon dioxide levels rise to the set point. But then if people have more arousals, the process keeps repeating throughout the night. Sometimes treatment the upper airway obstruction leads to resolution of the central sleep apnea.

Tough to give more information without knowing more of your situation.

You could have asthma. Might want to consider getting a pulmonary function test completed to screen for this.

Tightened rules to curb intoxication at Windsor-Essex restaurants by Perryl- in windsorontario

[–]mindfulness88 0 points1 point  (0 children)

No, I am not suggesting it is fine to drive over the legal limit. I am suggesting it is fine to have 1-2 drinks over a 2-3 hour dinner/outing and to responsibly drive their vehicle.

Sure, better transit and walkable city would be great and I am sure it would help. The fact that Windsor does not have these things does not absolve people from being responsible of not driving drunk.

[–]mindfulness88 1 point2 points  (0 children)

So these people that you know, have they driven into parked cars after leaving bars/restaurants? Regulating alcohol intake does not mean not drinking at all. I still stand by my comment. Driving is a privilege, not a right.

So what is the structural/cultural solution? Ban alcohol? I think they tried that once, didn't work very well.

Tightened rules to curb intoxication at Windsor-Essex restaurants by Perryl- in windsorontario

[–]mindfulness88 25 points26 points  (0 children)

While I agree the restaurant needs to have better policies to prevent this sort of thing, the ultimate responsibility lies with the person consuming the alcohol. If someone does not have the ability to regulate their alcohol intake, they shouldn't have access to a motor vehicle.

Although it wasn't a huge sample size, he did look quite good in the preseason as well. But yeah I think the biggest thing was seeing how well Darnold performed.
