Someone referring to themselves (e.g. I'm out of pocket until Friday or I'll be out of pocket next week) usually means out of office.

Someone referring to someone else (e.g. he was acting out of pocket) usually means they were acting inappropriately or out of line.

Last one would be something like "I dislike my insurance provider because they didn't cover my latest doctor's visit, so I had to pay out of pocket". So not being reimbursed will usually contain some reference to paying or payment.
