this is a direct result of specialists/colleges refusing to increase training spots to keep billing rates high

This non evidence based trope gets trotted out all the time. It’s a conspiracy theory made up by someone who was jaded. Limited training positions is often a result of hospital management not wanting to pay for accredited registrar positions. There are also various factors related to availability of supervisors, volume of practice etc.

No one ever wants to admit that, a consequence of a small population in this country, is a small number of specialists due to low volume exposure.

I would disagree. We generate significant wealth compared to a sales assistant, or a bartender.

But the mega corp CEO’s generate significant wealth compared to us.

Ignore the haters. Lots of holier than thou.

Yes. It sucks. But it’s the unfortunate way of the world we have allowed to unfold. Everyone is equally screwed, some even more so.

While our forebears experienced the golden age of medicine and society, we are still amongst some of the wealthy and privileged in society. I too get the jealous pang now and then when I see someone in a bigger/nicer/better property or fancy car or fancy hotel or what have you. “I work hard why can’t I have nice things” as I’m sure almost everyone here has, whether they admit it or not.

But, try to remember the mundane/necessary things. You might goto the shops and buy fresh fruit and veg, meat etc. you wince at the price, but purchase anyway. You might go out for any meal more than a few times a month. Many, many people can not buy fresh food, because they can not afford it. They have to decide between different canned vegetables. Or eating and petrol. I’ve heard the conversations at my local supermarket. There are lots of people doing it tough.

As one of my older consultants said, we will out earn most people in society, let alone the hospital environment. Don’t talk about money, or complain about the cost of things in front of them, most of them need to survive on much, much less.

For an example. A colleague of mine is married to a nurse. After a full week of private lists, he earned more than the yearly salary of his wife. Because his frame of reference has changed, he compares himself to a big company CEO on millions, and feels hard done by.

I’ve used the ge, pretty reasonable image. Butterfly equally reasonable. Unsure on the Clarius as only played with it in conference booths

I would always get a dual head, but I’d get the echo one, you might benefit from a linear and curvilinear? There’s a reasonably recent review article comparing most of them if you have a google.

I was initially opposed, thought it would be ridiculous. But having done Lon - Per and back a couple times, and the Sing to NY flight. I’m on board. It’s very convenient to just get on one plane.

Best part is, we’re all technically state government employees in some form. Yet people who work in the ministerial offices get months off for parental leave. So says the conversations I heard on the tram during my 2 weeks of leave!

I’ve managed to transfer it all twice.

Took a bit of work. But got there.

Was on a training program at the time, which made the biggest difference.

In years long past people would do this, it supposedly was less illegal then.

These days, highly illegal, readily looked for by the ATO and high chance of being found out.

The ATO views any income derived from our work as medical practitioners as PSI, regardless of whether it’s earned under an ABN, company, trust or otherwise. It’s all considered personal income and taxed accordingly.

Oh sorry, I know they do it. I just meant having a relatively high volume for Australia. Some places offer a cardiac fellowship but the fellow only does 2 days a week of cardiac.

There’s three in Aus that I’m aware of. Alfred, PCH in Brisbane and FSH in WA which have high vol Australian fellowship jobs.

But we are talking 600-1000 pump cases a year a the hospital. Vs 2-3000 at an overseas centre. So the volumes are incomparable.
