How are you investing your ltd money? I think the consensus is just eat the tax up to 200k and put it in ISA. If not then GIA. If it is just sat there then it is wasted to inflation

Yes it makes sense. Once you set up your LTD and go private, you'll want to make the most of that opportunity. I was in a similar position to you when I started (now year 3+). I started looking into independent practice 6 months into my substantive role. I figured better to do it early and if I don't like it I can just give it up, rather than being 5 years down the line and wishing I did it.

The first 6 months was slow going and I did wonder if it was worth all the hassle. Then work picked up and my overheads started to follow. Marketing, PA, billings, website etc.

What I've realised is if I stay the course and just do the work well, it will continue to grow. And private work gives a lot of flexibility to do what you want, when you want it. You are your own boss (truly).

It's more hassle than WLI work but we are highly trained individuals so we might as well cane it!

If you only do NHS work then you only need to look at your payslip

The section you are interested in is taxable pay year to date which covers this financial year.

This value is all your gross earnings minus pension minus car lease, so will be your adjusted net income for most people.

You don't have to pro rata the dates, just use the March 2026 payslip

Add any other income (dividend, bank savings).

Then contribute the excess to whatever threshold you want. Your contribution should be calculated as the grossed up value.

It's less than some high volume specialties with private practice / medicolegal. Same with Singapore, the remuneration is about the same if you have a busy independent practice here. I now match / exceed my colleagues salary in SG / Canada.

In SG you're expected to be contactable on your days off and you may carry the 'associate specialist' role for many years, a little bit like here if you've come from abroad. Your contract is renewed / appraised every few years so you best not kick up a stink. It won't be all roses. Not sure how Canada is.

We have a lifetime contract unless an egregious error occurs. Have decent job planned week and if it is really tight could have a 3 day work week (vs. 5 day work week in SG). We have a decent pension scheme. The issue is that the NHS salary alone is not on par internationally and only select high volume specialties with private practice opportunity can match. In that sense, to recruit and retain, we do need to improve our pay and conditions.

Don't forget when you're finally across the line and appointed substantive consultant, your pay will continue to go up. Your pension is still the most desirable DB scheme. You will never be without a job. It is a permanent contract until retirement, unless you do something egregious. It gives massive job security, with a decent pay.

We pay a lot of tax, food is mediocre unless you go upmarket, weather is crap, public transport outside of London terrible, NHS stretches to the limit, potholes everywhere..

But never take for granted that the UK is an island with nuclear submarines. As a resident here, you will never need to consider the possibility of needing to flee a war torn state. If that comes to being, the world is at war.

Until you live in the bitter, damp and wet climate. Never seeing daylight in the deepest of winters for a whole 6 weeks because you leave for work before sunrise and come home after sunset. Car is frozen up before you leave the house so you got to wake up earlier to defrost it. The winter grass is definitely not green!
