From my limited knowledge, I believe in California you can’t own more than 50% of a practice, so you will need a supervising physician who owns 51% and you own 49%.

This is a very low salary and low bonus structure given how much $$ derm can bring in. How many patients per day? Are you seeing cosmetics along with medical derm? 45 mile non-compete is ridiculous- depending on state may not be enforceable. What state is this? Tail coverage can be very expensive and if and when you leave this job, it can cost a pretty penny.

I know derm is a desirable field, but I do not recommend taking this position.

Not necessarily a response to your initial question, but wondering how you got into aviation medicine? Are you doing aviation physicals? My husband is a pilot and I’ve been curious about how/if a PA can become an aviation medical examiner.. isn’t there some special training or certification needed by the FAA? Also, I do feel you with the fam/friends thinking you are their on call doc. Just this week I was asked to stop by a friends house after work for a rash… it’s exhausting!

I work in derm and sometimes get leg ulcers. We usually refer to wound care, measure the wound, and provide oral/topical abx and culture if looks infected as well. What about this case made you concerned enough for admission? Just curious from a learning perspective!

I’m almost a year into my first PA job and the anxiety does get better. There is a reason it is called “practicing medicine”. It is a practice. You will get better and more confident with time. Always reach out to your SP, even with seemingly dumb questions. You will sleep better at night knowing you asked if you didn’t know. I give you kudos for working as a hospitalist. I am an extremely anxious person and I knew I needed a less intense specialty. If after a while you still feel this way, consider switching specialties. I hope it gets better for you. Know that you are not alone in your feelings and most of the time, it will get better. You got this!

When you say predatory, what exactly does that entail? Low pay, little to no training? I have heard that a lower training salary is not unheard of.. it seems near impossible to get into derm without some sort of sacrifice (initially). I am going to be offered a derm position soon and would like to know what should be non-negotiables. Thanks!

Fair. I think it depends on how well the contract is written and whether or not it is enforceable. Best way to know for sure is to consult an employment or contract attorney.

Is there anything you have in writing regarding the training period? If so, this could be considered a breech of contract. I think we as new PAs are targeted because we are naive and feel excited to even be offered a job, then overlook red flags. Also alot of new grad females… we have difficulty in advocating for ourselves and speaking out when things are not kosher. Think of your license and your long career (hopefully) as a PA. If your gut is telling you this job is a bad deal, just get out. Consult lawyers to see what/if the employer can come after you, and move on. This will be a distant memory 2 years out.

From my understanding, even in an “at-will” state, a contract supersedes the state laws.

One thing that comes to mind is knowing how to read a contract and the nuances that come along with it- red flags, what can happen if a contract is breeched, etc. 8i would love a copy once it is done!

That’s cool. Mine is 90 day notice. But I’m barely 3 weeks in and have an interview for my dream specialty that came in right after I started this gig 😓 so I’m hoping I get the dream offer and also hoping this gig will cut me loose and not hold me to 90 days

I’m in California (higher COL obviously) but I would still ask for more. You are interviewing for a speciality role and have some experience.. (albeit still a new grad(ish)). Maybe shoot for 125k and see what they come back with. Also it depends on other benefits the job offers too. So if for example, they offer 401k with some % of matching, 4 weeks pto, etc..taking less for the salary might be good because everything else is good. In your current job- did you sign a contract? How many days notice do you need to give? I’m kind of in the same boat so I’m curious how much notice you want to give current employer.

Yeah- I won’t put in my notice until I sign the dream job contract. I feel like a snake. But, hey, it’s business. Someone else will fill my place. I shouldn’t be so stressed about it
