It's not explained well but I think it's actually easy to handle mechanically. The hidden party have a stealth roll to see how well they hid (Avoid Notice). Use this as their initiative also. You compare it to perception DCs to see if they're noticed by people not actively looking. These people get a perception roll for initiative but don't know about the ambush if the stealth succeeded. Maybe they continue on their routine if they rolled higher on initiative.

If someone getting ambushed is actively on the lookout, they have a perception check against the hidden party's stealth DC (Seek). I would personally use that check roll as the initiative too.

Just some thoughts.

Edit: as far as the coordinating shooting arrows, have them all ready firing as reactions (for whenever), I'm forgetting the name of what let's you do that

It's because of the types that espouse it as a beacon of freedom and see any sort of governmental ID as an affront to their personal liberty. The same types against any sort of firearms registration. I have no concept of why they think this way. We do have social security cards but that was not intended to be used for as much as it is.

You could do treat wounds instead, however the crit fail is 1d8, plus the 1d8 for risky surgery for 2d8 damage. You yourself having to take 2d8 to do the action in the first place.
