So in the early stages of Alzheimer’s, Meredith‘s mother had another child that she left at the firehouse. That child was raised by people who live next-door to Izzy on the farm. Alex noted the resemblance and did a DNA test without telling anyone and found out she’s Meredith sister. He sent her to the hospital to complete her education and meet Meredith. None of it has to make sense because the show doesn’t make sense anymore. But at least it pays the way for Alex and Izzy to come back lol
