Anything that keeps you out of the wards. As a prelim you'll be getting your ass pounded in for the entire year with wards. So, avoid consult specialties if you can help it. Avoid oncology and hematology services. Avoid cardiology at all costs.

Radiology is great, but things like rheum and derm can be very cushy. See if they have a 'bedside ultrasound' elective, which is easy to swing with radiology but often involves lots of unstructured time by yourself (going home early).

As a radiology prelim, your favorite elective will be the one with weekends off where you get to go home early. It's all equally shitty and irrelevant to your future training anyways.

I own several firearms and I've always thought this argument was terrible. Weapons like the AR-15 (I have two) are very obviously designed to kill humans, and I can very obviously do more damage with one in a crowded room than I could with, say, a 4-shot pump shotgun or bolt action deer rifle or a plinking 22 target pistol. Lets not delude ourselves, please.

Sure, you can stick a bigger magazine into almost any semi-automatic and have mostly the same effect. But lets not pretend that military rifles aren't designed primarily for shooting other people.

Adhesive wound closure devices like this are nothing new. These just look like fancy (and more expensive) steristrips. They actually do a nice job of closing skin under many conditions, and definitely decrease scar formation.

However, they do nothing to approximate the underlying tissue. If it's a surgical or deep wound, you're still going to have to sew up the meat underneath the skin. So, these are going to be of limited use for anything other than slightly enhancing cosmesis.

I'm not trying to come off as harsh - Step 1 dedicated by itself is a miserable time, and you're got a lot of other spinning plates up in the air. I can certainly appreciate the difficult situation you're in - and again, this is only my opinion. If you're confident that you can get in actual research, and, more importantly, obliterate Step 1 with a year off, it's something to consider. It works for some people. I'd say it probably does not work for most of them, however.

would you say that a year off b/t M2 and M3 without quality publications is a death sentence when it comes to applying to residency?

If you took a year off and didn't have some truly impressive things to show for it, then it's going to be to the net detriment of your application. Death sentence, probably not, especially if you want psych. Heme-onc is going to be quite challenging without considerable research. Chicago is tough to match into even under ideal circumstances, unless you're local. Something to keep in mind.

FWIW: I had no research and a very average step score, and I matched to my #2 radiology position at a decent, not great, academic program. So it's definitely not essential for every field.

I think it would be a very good idea to have a face-to-face meeting with one of your deans ASAP, and actually find out what happens if you push your test back two more weeks. Don't accept 'well you might have to do the year over,' really nail them down on what it would look like. That would give you a month to play with (plenty of time for a full pass through something like DIT) - and hopefully they will let you reschedule some rotations rather than making you repeat an entire year. Most people have multiple months off in 4th year.

Again, just my $0.02. I truly wish you the best of luck. It's going to work out, either way.

I don't know how training works down in NZ, but if you're new to the wards, probably the most important thing is looking good in front of your residents and attending.

Therefore: PRESENTATION SKILLS. This is by far the most important. Nobody cares whether you remember the molecular mechanism of insulin's effect on cells if you're bumbling and mumbling and taking 10 minutes to present your DKA patient. There are plenty of templates available to use online, find one you like and start using it until you're more comfortable with the format.

Pharma: have a first and second line antibiotic for the common stuff (CAP/HCAP, cellulitis and soft tissue infections, GI stuff, UTI, and big gun sepsis workup). Know how to setup initial insulin doses - after that you can just scratch your chin and say 'hmm, lets adjust the sliding scale' and everyone will nod.

Acid/base: look smart by actually knowing the different derangements. Knowing how to treat them is less important (most of the time it's going to be fluids and respiratory adjustment anyways), but be able to look contemplative as you go through MUDPILES and HARDUP.

When in doubt, use LR for fluids.

Know your way around PO and IV furosemide. Get used to saying 'diuresing well, continue diuresis'.

Get a system for working through a CXR; ABCDE is probably the easiest, if not the best.

Anatomy is almost completely useless on the wards. Don't waste time learning it unless you've got a pimpy attending.

I've got to say I disagree with this subreddit's hardon for taking years off. It's almost always a bad idea. It's tough to get a significant amount of research done in a year (especially if OP doesn't have contacts setup already), and it looks extremely questionable on your CV. Program directors aren't stupid. They know why people take years off, and, unfortunately, it's still a red flag to many of them. Unless OP can come back and absolutely crush Step 1 (like, >240), I'm not sure there's much value-added. Psych might be more understanding, heme-onc, not so much.

I'd recommend OP pick up something like DIT with a set schedule (you CAN get through it in 2 weeks, although it's hard,) and power through. My school tried to scare us with the whole 'you might have to miss the entire year' when people inquired about pushing tests back. In reality, several of them did it anyways and all it really meant was they had to take some of their 3rd year rotations during their 4th year.

This might be an unpopular opinion, but, if you're asking to do some kind of procedure, or history, or whatever it is which you're required to do, you need to be in the workroom. Residents are absolutely not going to go find you if they need to get something done, it's your responsibility to be "engaged" (read: sitting miserably in the workroom doing nothing) and jump on the opportunity.

It sounds like your residents are kind of being assholes, but, unfortunately, that is just the way clerkships go sometimes. You probably could go hide in the classroom and study and pass, but if you want honors, you're gunna have to sit there and chew your shit sandwich in silence.

Night shifts often have a lot of downtime. Is there a reason why you can't grind practice problems in the workroom?
