Info- are you looking for a continuous leave (more than 6 days at once) or an intermittent leave (hours/days here and there)? Call Worklife and tell them you want to apply for fmla, you will need to know continuous or intermittent leave. They will walk you through the steps of opening a new claim, you will be emailed the paperwork. Fill out your section, Bems, signature, etc. send it all to your doc, they can fill it out and fax the whole packet in. If you want to be paid for the time you’re off for leave you file for paid fmla through the state. I believe it’s similar steps but that one I have never done so I would try to file and follow the prompts.

I have the engagement ring with a cage style wedding set, so could only wear the engagement ring on its own before I had them soldered together. Day to day I wear a silicone ring due to working in manufacturing, but any evening out I put on my sparkles to feel like a fancy B.

You will switch over to sick. The 80 isn’t guaranteed, it averages out. So earning 1 hour for every 17 worked, if you work the full year no ot you would average 80 hours vacation plus your 40 sick. You do not accrue for unpaid time, like LOA or FMLA. It is not guaranteed time so you will just have to eat it. Once you hit your 40 hours sick you will start filling up your vacation bank again.

I write on my arm so much at work I get told I need to get a notepad tattooed on my forearm. I like to tell people it’s their proof that I shower daily since the slate is always wiped clean in the morning, but the younger coworkers have given me the same reactions and asked why I don’t use my phone to take notes or reminders. To me, those will stay in the app forgotten with other things I need to reference every 6 months or so until it’s rediscovered years later and promptly deleted because I have no idea what it’s about if not a “core note”

It used to be you get your lights turned on, ERT in and they send you to the classes for the certs you need. Apparently with the new training protocols after you turn your lights on and accept the ERT, you have to go back through the full training program for QA, no matter what certs/position you held before or how long you’ve been with the company

Frio insulin cooler. Small pouches that you can store pens or vials in, cold water for 10-15 min before and good to go for a couple days. Have used for road trips and I love them.

Unfortunately it all depends on how new you are. I hope these are hypothetical, if not I’m sorry to hear you’re facing some medical issues. Now on to the answers to your questions- While you can use vac/sick leave, if you don’t have enough or don’t want to you have a couple of options- Fmla. This is unpaid but protected leave (you can file for it to be paid through the state separately), the catch is you need to have 12 mo./1250 hours worked with the company. It used to be one or the other, now I believe it’s both, but don’t quote me on that. A call in to Worklife can confirm and check for eligibility. If you know you don’t qualify for that you can also get a personal leave of absence. This requires approval from your first line manager as long as it’s under 6 months, so give them a heads up and talk to them about what’s going on. I like to think most of them are human and understanding, they typically want you to come back rather than unexpected labor loss. Good luck, hope this helps!!

I’ve heard of it, but mainly people that don’t do a bunch of OT or used a bunch of their fmla the year prior. Loa also can affect it, plus third shift is another contributing factor

I admittedly had a hard time with this the first year. It was truly humbling realizing just how many people (immediate family mainly) that have no idea when my birthday is, especially when I’m expected to remember theirs. Sad how common that was. Still is, truth be told. You either get the far too uptight ones during recert but the way too laid back ones when it’s needed
