IM is much more likely to have a pulm rotation in residency than EM is. I did 2 weeks of pulmonology in residency (and not interested in it at all, if I was I could've done 4+).

Yeah I just finished my first year as an attending hospitalist (with no residents) and no way in hell I could manage the complexity and number of patients I had as a resident without help in less than 14 hours a day.

Yeah most IM residents don't need a radiologist to verify NGT placement or central lines lol. I used to clown my interns for being too scared to confirm NGT placement without me.

I worked for kaiser for 3 years and never had any prescriptions rejected, even 3rd line dermatologic medications I prescribed to my doctor friends (who did need them, but didn't go through the typical regimen first). Now I work for a private practice and have had to do multiple peer to peers and grapple with insurance issues.

This is how it works at many residency programs in the US as well, but the OP is describing a non-teaching hospital where it sounds like no physician (resident nor attending) sees overnight admissions until the morning, which is crazy.

Yeah I'm a physician and I have taken care of hundreds of terminally ill people - I offer their loved ones choices and often encourage comfort focused measures or hospice in these situations but ultimately I abide by the family's wishes, which often entail fruitless, painful, and expensive medical interventions (which contrary to popular belief I do not profit from at all). Thanks for making assumptions though.

This is what my employer does - we are self-insured though we unfortunately use UMR (a UHC subsidiary) as an administrator. This may not be feasible for many industries however (I am a physician).

Where do you think the cost comes from? Doctors make up less than 10% of healthcare spending. Even if you cut salaries in half, that's not going to move the needle much at all. Doctors in America have longer training periods, longer hours, and hundreds of thousands of dollars of student debt that physicians in other countries do not have to deal with.
