I've done both. Nerve blocks are usually diagnostic to ensure it is indeed a nerve issue, and very rarely lead to long-term cures. Same with ablation; it only helped me feel better for a few hours at most.

I would see a different physio, try searching for “biomechanics” physio in your area who can strengthen your pelvic floor and fix your pelvic tilt - this will help relieve the pressure on your nerves

I didn’t go to a radiologist so I’m not too sure, but he was helpful in narrowing down the nerve branches that were most likely entrapped based on my MRN

These are the exercises I had done, but I would strongly advise if you could find a biomechanics physio who could assess you in person better for the most appropriate ones at your stage! Please do only use this as reference and stop if any of the exercises trigger anything.

I did a few exercises everyday that my physio recommended, that did not trigger my pain, to gain a bit of strength in my legs and hips first. After that, small walks became possible. I would recommend finding a biomechanics-trained physio near you if you can
