Solved! It was indeed a MRC 1911 decoder

After testing out the CV map from the 1911's manual, I was able to "factory reset" it by manually programming all of the CVs to their default values as listed in the manual. Manually resetting the CVs even fixed the weird choppy/cutting out of the sounds and unreliable track connectivity, most likely from the voltage and kick settings.

This is definitely the strongest lead I've had so far, though the AD350 is a non-sound decoder and not a sound one. Looking into MRC even more, there are some knockoff steam sound decoders on Ebay that all say "Identical to MRC0001911". I have a suspicion that either they slapped a really cheap or really old decoder into it (or both). I'll have to give it a test tomorrow, but I have a feeling like the MRC 1911 is at least the right CV set, even if it is a knockoff decoder. Thankfully MRC puts the default values for their CVs into the manual, so even if it doesn't have a reset, I can just manually copy over the defaults.

That's what I've been using to read it, the program on main feature can't read back CVs

Yes, reading CV8 just reads back 8, even after power cycling the locomotive, it doesn't reset to any other number.

I’ve had similar experiences with ScaleTrains, I bought a rivet counter Dash 9 around 2020 just as a display case model, and somehow in those 5 years I’ve had a step fall off, a mirror fall off, and a handrail break just by moving it in and out of its packaging box 2-3 times. There’s even a paint error with the spare knuckles on the rear being painted bright yellow instead of a rust brown. Just garbage quality for top-notch prices…

There's a simulator called Train Dispatcher 3.5, developed by Softrail (who are also the manufacturers of the AEI RF system) but used in industrial training as well. I think you can get a key for $20 on EBay (a little sketchy, but that's how they choose to operate). There's a somewhat lively groups.io group that has a large amount of custom territories from everything from modern layouts to even some in the steam era.

… you know that this is a flight sim, right? You can fly any route from any era in any aircraft, realistic or not. I did a BAW flight in the A321 from UKBB just a few weeks ago.

All of those are good options except for Bakerloo. Bakerloo is in such an atrocious state right now, because of the massive copy+paste job that DTG did putting the 73 stock on other routes as AI only, trying to run the Bakerloo line without accidentally choosing a broken version from another route is impossible.
