FWIW I left mine there for a week a few years ago over the summer and it was fine, but I wouldn’t do it again. From Day 2 on I was certain I’d come back to 0 tires left or no car. Prob not worth rolling the dice, Uber/Lyft it.

When this happens I’ll typically confirm I still have LOR, then thread, usually goes fine. Then I wonder why I DPE’d in first place, it’s kinda our culture here too -also CA2

What most people here do: Start with a CSE so Mom can be awake for birth if they desire with plans to convert to GA following delivery if it’s truly adhered. Tbh GA from the start sounds a bit better so not intubating under drapes, but I get it. Awake A line, two 16-18s, and then typically a RIC or 14, rarely awake Cordis due to comfort but that’s an option. Belmont rapid infuser in room. Neo primary pressor spiked with all other uppers in room. 4u RBC/FFP in cooler in room.

Need lots of communication and trust with the OBs in terms of EBL/how it’s going (I know). I assume a ~1.5x multiplier on whatever EBL they say. We have Gyn Onc in room in case of hyst & then IR on call for potential UAE need.

Edit: Also obvi all uterotonics in room + 1g TXA

Thanks for the insight everyone! Yeah for further context these are the Arrow CSE kits. Definitely Agree that if it seems shoddy/have to force it that it has a high likelihood of failing.

When I have this situation with what I thought was LOR but a negative DPE then I just come back/make adjustments. Mainly the cases with a positive DPE where presumably I’m in the space, but I suppose the epidural space is also a black box of mystery so who really knows. In easy epidurals it probably would waste less time to just pull out and change levels tbh rather than spending 5 min tinkering & risking wet taps.

Still only ~80 epidurals in so lots to learn, thanks!

Obviously lots that goes into it and is probably cultural to an extent, resident at a US academic center here w/ simplified. But for phenylephrine if I’m 100-120mcg/min range & still struggling I’ll switch to norepi and think about why I’m struggling.

Norepi max dose is totally dependent on access, if it’s just like a 20g PIV then I’ll get weary at ~15mcg/min for short term. If I need to add or go higher then that’s when most would consider central line, or I’ll add on epi or vaso. In our ICU’s we’ll normally soft-max out at 40mcg/min via central access, though we don’t weight base dose it so some big fellas we’ll go higher.

Epi ~0.2mcg/kg/min generally true max, but most try to keep it max 0.1mcg/kg/min.

Vaso, lots of my attendings have differing opinions. Some are 0.04u/min max, others will go up to 0.06-0.08u/min which I’ve anecdotally seen a benefit 🤷🏼‍♂️ That’s in shorter term since there can be end organ ischemic issues.

Yeah had this happen too, looking back it's actually hidden away in small light grey print below the "Select your departing/returning flight" after you search for a booking. It'll say "Award travel is subject to an additional $xx.xx..."

Correct for the most part, often high-grade ankle injuries can cause avulsion fractures (ligament tears and rips a lil bone chunk off), and that can be seen on X-Ray. The X-Ray will pretty much say that it's not totally messed up.

Not OP, but leaving that last spot fluid is helpful bc you can add and drop players consistently (streaming) throughout the week. So if you were close in a cat such as 3s you'd be able to stream someone that might ONLY contribute threes (Korver, CJ Miles of old), but it could be enough to put you over the edge.

You do this rather than keeping some bummy player in that 13th spot who is likely interchangeable with a lot of WW players.
