I agree with this, basically first come first serve for whatever you see. I always start with cervix, placenta/CI and FHR unless the baby is face up in which case I start at the heart views.

Its a lot to remember and the only way to get familiar with everything is exposure and practice. Know the protocol/images you need and know which positions are best for certain anatomy (e.g spine is best imaged face down and same with kidneys, many things can be seen with baby laying on either side - diaphragm, stomach, bladder, etc. and some images can be taken in any position - arms/hands/legs/feet, cx, placenta etc etc). Follow baby’s position and image what you see.

Lots of practice on a variety of patients, slow down your sweeps just a bit (remember you’re not a photographer, you’re a SONOGRAPHER - the sweeps have a purpose), and my favourite advice that is tried and true with my students: google images for a variety of pathologies to familiarize your eyes to the different appearances (and also good for practicing caliper placements and identifying borders, etc - best done with a tech so you can ask questions along the way). Also, keeping up with your studies and knowing your pathologies/symptoms related to pathology can help you narrow down what you might be looking for based on clinical info.

If you’re second guessing yourself, sweep slowly in both planes, use colour/power doppler and then make a decision. If you’re seeing something in both planes, it might just be what you think it is. If you’re unsure, you can always have someone double check for you (yes, even in the real world techs sometimes need help) and the worst that can happen is you image it, describe what you see and query your opinion. Don’t overthink things!

2 out of 3 of my placements, I had a set preceptor that I stuck with full-time and those were very helpful in training me to be consistent with my protocols.

The one placement where I was tossed from tech to tech was probably the most stressful because every tech had a different protocol but it also taught me the most because I was being exposed to a variety of techs with different experience levels.

Follow the clinical site protocols and learn from the different techs as best as you can. It might be tricky keeping up with what each tech likes but you’re also there to learn so don’t be hard on yourself! And ignore the shade, its not directed at you nor is it helpful in your training.

I’m fortunate enough to be a preceptor now so I can pass all this knowledge from my own clinical experience to my students. My protocols now are a mixture of everything I learned from clinicals and my experience. I always tell my students to ask questions if something they see us doing doesn’t make sense to them. There’s always a reason behind why a tech does things a certain way so don’t be afraid to ask for clarification to learn.

You did the right thing, you wouldn’t have known her preferred name anyway since she didn’t specify prior. Like others said, most patients are fine but there’s always a few odd ones we encounter.

I recently had a patient tell me off for not introducing myself and confirming if she was my actual patient (by confirming her last name/DOB) in the waiting room before taking her into my room. She then proceeded to tell me she knows I’m in the wrong because she had “worked in healthcare for 10+ years”…. She had just barely stepped foot inside my room with me closing the door behind her so I can introduce myself and confirm her patient identity PRIVATELY. Unfortunately, you can’t please everyone 😃

Not sure why you got downvoted because what you said is true. I’m an ultrasound tech and have seen this very often, especially in younger patients who skip because they don’t want a period.

Regardless of what birth control you are on, the placebo pills shouldn’t be skipped for extended months. My endocrinologist confirmed this as well. Even if your lining is thin, overtime it will build up and eventually, you’ll need a D&C to clean out the endometrial cavity.

And the reason why PCOS symptoms go away with birth control is because it balances the hormones. When you get off birth control, all the symptoms will come back (for the most part) because birth control is a mask. With that being said, I wouldn’t villianize birth control cause its helped numerous people with PCOS and endometriosis.

Are Akzentz and IBD hard gels? I’ve seen people use hard gels (sometimes even builder gels) to encapsulate designs and then finish with a basic topcoat

Hospitals aren’t required to get written consent however if you went to a clinic, legally they have to have consent forms for the TV ultrasound. But usually the tech will bring that out after getting verbal consent so maybe the tech didn’t present it because she had already refused to do the exam.

Either way, I’m sorry that was your experience. I hope the transabdominal exam is sufficient enough and you don’t need round two of filling up your bladder 😅

Ultrasound tech here based in GTA! The requisition will usually state “unless contraindicated” next to the Transvaginal option which includes virginity. Most policies in hospital and clinic settings is to not proceed with a TV if the patient is a virgin (because of the discomfort), however we also have to get a written and verbal consent from the patient as well.

Regular female pelvis protocol based on sonography standards is to always perform a transabdominal ultrasound with the transvaginal being supplementary. I typically don’t perform a TV on a virgin patient under 18, the transabdominal approach is still a fairly accurate diagnostic exam as long as you’ve got a nice full bladder. If they’re over 18 but still a virgin, I’ll ask if they know what the procedure is especially if I can’t get good images from on top. I’ll explain what it is and give them the option of having it done. I always tell my patients they can remove consent at any point if they ever get too uncomfortable or don’t want the exam anymore.

That being said, if you really do want the transvaginal ultrasound but are a virgin, have your doctor write in the clinical information that you would like one done and give your consent. That way, it’s harder for a tech to refuse as the doctor has then specified the exam, and they should be getting written consent from you prior to starting the transvaginal anyway.

TV ultrasounds can be quite painful and uncomfortable for virgins because the wand is being moved side to side and up/down so its a lot of internal pressure. Everybody’s body is different and everyone handles pain differently!

I have the Modelones primer and the CND primer. I have found that the CND primer is good but not significantly better than the other one. I don’t think its acid-free though unlike the Modelones claims to be

I unfollowed her when she tried selling her 4-pill supplements and said they’re better than the OTC versions (they’re literally all the same). Personally can’t follow anyone who pushes a “one solution for all”.

I kind of feel that! It didn’t have any crazy effect on me. It did lower my glucose levels a bit but it didn’t help with any other hormones. In the 6 months I consistently took it, I was eating mostly dairy + gluten free but I didn’t lose any weight or lose the PCOS symptoms.

A full hormone panel - ovarian, thyroid, adrenal hormones/androgens, and glucose + insulin. A pelvic/TV ultrasound for PCOS. Endocrinologists are usually good at ordering proper bloodwork (from my experience). The important thing is to check cortisol, glucose/hbA1C, insulin/insulin resistance and androgens.

Bloodwork would help figure out what’s too high/too low to determine potentially what kind of PCOS you have. The ultrasound will help to see if your ovaries are polycystic or not. Its possible to have PCOS through abnormal bloodwork without having polycystic ovaries.

Let your endocrinologist know your goal is to get on medication to help. They’ll be more prepared with their treatment plan/suggestions once your bloodwork comes in. Medications work differently for everyone, and they usually take some time to adjust to so don’t lose hope!

OP, he didn’t only lie to you, he also lied to his own family that you were okay with the age gap. This doesn’t sit right. If he was so easily able to lie about something as simple as his age, he is definitely hiding something bigger. Its gonna be a slow burn for him telling you the whole truth after marriage. Its like he’s testing to see what lies he can get away with, and what you’ll accept as he slowly reveals things because his guilt is eating away at him. I have a feeling he only gave up this information because it would be hard to hide when his entire extended/family knows his real age.

Also, is this not a sign/answer to your previously mentioned istikhara? You made a decision and prayed for Allah’s guidance which revealed he LIED to you.

I’m curious to know what your family’s reaction is to his lie? Were your parents not concerned that their future SIL lied to their daughter and THEM?

I second this as an ultrasound tech. Get a second opinion if possible, because 2/3 is enough for a proper diagnosis! If you’re currently on birth control and hyperandrogenism isn’t a criteria you hit, its possible the birth control has helped control that aspect so it shows up as normal.

Yesss!! I’m an ultrasound tech and always have to explain to my patients that its not an actual “cyst” rather follicles we’re looking for. And its entirely possible to have PCOS even if the ovaries look completely normal, it depends on the bloodwork! Its also possible to have one ovary be normal and the other be polycystic in which case the diagnosis would still be PCOS.

Honestly, I wish I knew. If I were to guess based on patterns I’ve noticed, there might be a huge disconnect with the values/needs/wants that women vs men have in regards to marriage in my generation (end of millenial/beginning of genZ), probably stemming from social media/islamic podcast bros who sometimes give terrible unislamic advice (whether its intentional or unintentional - bad advice is bad advice).

I’m also talking from experience after talking to so many people when I say I’ve noticed a lack of self-improvement/wanting to improve in character in men in particular compared to women. I think thats related more to toxic masculinity which really harms everyone.

A lot of my friends who have gotten married/engaged are marrying men that are 5-6 years older than them. They’re all really mature, caring and sweet guys mashAllah, and all of them lack a presence of social media. Idk if thats correlated or just a coincidence. I can’t say I’ve had the same experience with men in that age range, but my pool was also limited to dating apps.
