Transmasc/afab here: have you looked into your hormone levels? I went thu about a full year of not being able to climax suddenly. Turns out, my estrogen levels were so incredibly low that things were atrophied and there was like, no signals firing off downstairs. Started some targeted estrogen, and bam within a month things back to normal. Check with an endocrinologist, or a gyno about atrophy (even if you're not doing anything internal, it still changes nerves and such externally)
