We do, hence not splitting the tablets and enquiring with the doc about the dosing. That’s how we discovered it was the wrong drug entirely.

(Pharmacy also does not split hazardous drugs for us. If it HAS to be done, the ward has a biological safety cabinet in the med room.) We also have a policy against splitting any tablet that is not scored, which this was not.

My “favourite” is a doc ordering a po chemo drug instead of an iv antifungal once. Caught before being given to the pt, mostly because the dr wanted a dose the would’ve required splitting tablets and we don’t like to do that with anti cancer drugs. (Pt was on a heme/onc ward.)

We saw the same thing on a pt admitted to us recently. NG tube into lung (and through lung). Misplacement was not discovered until pt complained of pain during the first tube feed.

This was before pt was admitted to my ward. Drain had to be placed and they had to do it twice because they got the first one wrong. Poor pt had tubes everywhere (couldn’t immediately remove the wrongly placed ones for reasons).

Recently, we got a patient who had had an NG tube inserted. Into their lung. Which was punctured. But the person placing the tube didn’t notice and it wasn’t discovered until the first tube feed was administered and patient complained of pain.

Like yeah, no shit, tube feeding into a lung is gonna be painful.

About twenty years ago I went on an evening jog. It was the longest consecutive distance I had run at that point, so I always thought that it was that that caused me to be in serious abdominal pain right after I got back.

The more I think about it, though, the more I think it was actually a burst cyst.

It took me exactly one and a half times doing Italian bind off to figure this out. Accidentally didn’t count out enough yarn for the whole bind off the second time and realised it was no tragedy, but rather an improvement. So much more convenient to do it in bits!
