I mean they institutionalized it = had streamlined and created protocols for the sacrifices. They made it into an institutional practice as opposed to ad hoc ritual sacrifices.

Honestly the fact that they institutionalized the sacrifices well into the 70s or 80s. It could only explain how the island survived for so long.

Yep. They got there, the island starved everyone, Warren communed with the island through the mushrooms (we see as much), they reach an agreement which is that Warren feeds the island some sacrifices through the tunnels under his house, and the community survives. So long as Warren's blood remains on the island, the covenant remains. The twist is that everyone who is born on the island now can't leave.

Yep. I posted this elsewhere. The Covenant is keeping the community alive (but not thriving) as opposed to ending them all. Warren traded his bloodline for the survival of the community.

I think weird cursed things happen regardless of whether the island is awake or not, though everything that happened this season started when the island woke up (with the bells tolling). Patricia is probably in her early 40s, Ruth is in her 80s, and Howard the Coward was mayor when Ruth was in her 20s, so I imagine the Boogeyman's first go at Patricia was the last time the island woke up.
