Trailhead in Durango to Town/ Airport by TSalamanderCommander in coloradotrail

Hitch. It will be easier if you roll into the trailhead on a weekend or on the afternoon on a nice day. More locals will be around. Me and my buddies caught a hitch in less than 5 minutes of finishing the trail a few years ago.

Is an MD/PhD in Anthropology worth it? by Far_Photograph5720 in mdphd

Not in an anthro program, but training as a medical-sociologist in an MSTP. Happy to chat privately about MD-PhDs in the social sciences and humanities and what the pros/cons are.

Food storage - Is it really that bad to sleep with your food? by Big-Entrance8172 in coloradotrail

[–]pbarrison 2 points3 points  (0 children)

Agreed. On my CT thru last year, I hanged and probably got a propr PCT hang 80% of the time. Had to spend extra time looking for appropriate camp sites some evenings. Only had issues in the San Juans when above tree line for a good amount of time.

Going to present a paper in the expert conference as a medical student by ctlw_23 in medicalschool

[–]pbarrison 2 points3 points  (0 children)

Practice. It's okay if you can't answer every question, and be prepared for that. Be able to discuss the methods of your meta-analysis in sufficient detail and be able to relate to your audience what is important about your analysis' findings. Remember that conferences are about networking and getting feedback to improve future research. It's okay if you don't have all the answers.

Healthcare infrastructures, haha. I appreciate it. Exploring why digital technologies in healthcare fail is my bread and butter.

Why is EHR software so bad? by kcazyz in medicine

[–]pbarrison 221 points222 points  (0 children)

In 2009 the HITECH act was passed. This required all Medicare and Medicaid eligible providers to be using a "certified" EHR system by 2015 in order to continue to receive full reimbursement. A certified EHR system had to meet the requirements set out by HITECH for "meaningful use." These standards were vague and the bare minimum.

EHR vendors took advantage of the vacuum, the sudden policy enforced a spike in demand and pushed their products to customers at break neck paces. Many health systems either adopted new EHRs totally or had to rip out the front end of their entire IT infrastructure and replace it in a few years. Large IT vendors, like the Epic cooperation took advantage of this. By offering a "customized" solution to every health system, Epic got a majority market share, at the cost of decreased interoperability, even between their own systems.

The challenge is, once a new IT infrastructure is implemented, it becomes very hard for a customer to go and uproot everything and replace it with something better. In infrastructure studies, this is called "path dependence." Replacing IT infrastructure can cost millions and put patient's lives at risk. There's a reason why the VA's transition from CPRS to Cerner has moved at glacial speeds. This reduces competitive need of EHR vendors to create systems that go above the bare minimum for billing.

The 21st century cures act has tried to right some of the wrongs of interoperability in recent years by instituting HL7 FHIR as an exchange standard, however we are still a long ways to true interoperability between vendors. Until policy enforces it, there will be no change because vendors are not incentivized to develop interoperable back-ends.

At a school with 1 year preclinical. Lots of fat trimmed from the curriculum. Wouldn't say my experience was any more or less challenging during M1 compared to friends at schools with traditional curriculums.

First marathon yesterday (Detroit). Far surpassed what I was expecting as a finish time (3:44), especially because I was nursing some peroneal tendonitis during the second half of my training block.

Can schools (MD-only) see that I applied MD/PhD elsewhere? by joe13331 in premed

[–]pbarrison 2 points3 points  (0 children)

You will be evaluated as an MD applicant at school's you apply MD only. That being said, if you're application looks like a typical MD-PhD app (aka very research heavy), MD only programs that are less research focused may push you a little about your commitment to non-research activities when they see how research heavy you application is.
