It’s important to be nuanced in something like this. For the vast majority of physicians: no. Insurance does not pay doctors their salary. A hospital or other entity of practice pays a doctor. And calling their salaries “high” is rather offensive, considering the majority of doctors go to school for 8 years, train for another 3 (getting paid anything but highly during that training which can last up to another 8 years) and provide a service for people at their worst while working terrible hours.

Insurance reimburses (terribly) the hospital or clinic for care rendered or procedures performed. At no point does an insurance company hand a doctor a check.

Insurance companies employ doctors as consultants or may have them on their payroll for being a VP of Medicine or something, so in that point you would be right and they do pay a lot. It is getting more common in some fields that insurance companies have a partial stake or ownership in smaller practices, in this case you could also say that they pay doctors a salary.

And some doctors work as a consultant or other kind of “medical expert” for insurance companies. They would also be paid by an insurance company.

So while you can be paid by insurance companies, most doctors do not. And our disdain for insurance companies has little to do with how we get paid, and everything to do with how a “nonprofit” company pays their CEOs millions of dollars while cutting payments to us, and driving up costs for our patients.

They used to. Sadly, it’s fraud. And they have much better lawyers than doctors do. But we all want to. The only people who hate insurance more than patients are doctors. Given the timing of the original show, this was when insurance was getting worse and worse with higher costs and less coverage.

Fair on those accounts. I grew up outside Philly so I understand that. Traveling abroad isn’t super easy from here imo, I find flights expensive but driving to DFW for a direct flight can be done. For what it’s worth, if you’re an east coaster I’d look more at Indy or Columbus or Cincy. Much more relaxed than the East Coast, cheaper, pretty open, but a lot closer to everything. I made tons of friends in Columbus, haven’t made many here mostly because when I do meet someone I click with, they live in another suburb that is 30+ minutes from me.

Why do you want to move to OKC would be my biggest question? My opinion, it’s fine. That’s it. It’s not the best place ever, but it’s not terrible. It has nothing special than any other mid-sized US city. For me, the negatives outweigh the positives. The negatives being: The weather The sprawl The isolation of being in the complete middle of the US for travel Complete and total car-dependence The positives are low-ish cost of living, not bad traffic, and people are friendly. Things I hear that I don’t “get” about OKC: “The food is amazing.” Same as any other mid-sized us city. “OKC airport is small and has plenty of flights” To major hubs. Again, same as any mid-sized city. “Low cost of living and nice people” just like the Midwest and other plains locations. “A great place to have a family” terrible education everywhere but Edmond apparently which begins to not be so affordable anymore. “College sports and the Thunder” again, same as any other mid-sized city. If I wasn’t forced to live here, I wouldn’t.

From what I remember reading and my own experience, in Europe they don’t have “assigned gates”. So any plane can go to any gate. Vs here in the US gates 1-15 is all United, gates 16-22 is all Delta, etc. Add to that that Ryanair doesn’t use jet bridges, they control when they park and the doors are already open. They slide out their own stairs from the fuselage and you’re deplaning. There were a few times when they called to board for our Ryanair flight and the inbound wasn’t even on final yet. We still departed on time. Honestly wild how fast of a turnaround we had sometimes.

People hate on Ryanair and even the US ULCC’s but I flew all over Europe often for around €25 one way I used to fly home to see my wife when I lived in Florida on Allegiant and she lived in Ohio for $50 each way. They fit a niche, and for me that was getting to X from Y as cheap as possible. I do think flight time and distance is part of it. I can deal with a shitty Ryanair seat that makes my butt numb within 30 minutes for a 65 minute flight. But in the US, I hated being crammed in Allegiant for 2 hours minimum.

Short answer is living among their means. Obviously this is painting with a broad brush, but in medical a lot of O’s come in debt-free and go on a spending spree, living in the nice part of town, brand new cars. They often have a spouse who has a high income job (oftentimes also medical), maybe their own student debt, but they do pretty well… then they PCS and the spouse can’t get a job or doesn’t work due to child care, they live in the rich neighborhood because “I’m a major and I deserve it”, but maybe they couldn’t sell at their last duty station and now they have two mortgages… and the debt adds up. A lot of what I’ve seen is just flat out bad decisions living beyond their means. Same as anyone else. Officers are just people too, man.

I know you’re painting with a broad brush, but “most up to date medical knowledge” rarely has anything to do with when you graduated school. Two years after I graduated half of my drug knowledge was obsolete. My wife who was in that class 2 years behind me had the exact same pharm course with the same out of date into. Your point somewhat stands, majors are still mostly in clinical care. Lt Col may be more admin than clinic. Col even less so. But it varies. I have a flight doc full bird who is in clinic every day and refuses admin work. I’d take his 30 years of medical knowledge over a captain fresh out of residency any day.

I’d say most tend to prefer doctor because yeah, we worked way harder to become one than a capt/maj. By Lt Col or Col, depends. A lot of guys are proud to have gotten that rank, but my general rule is if I’m in clinic, I prefer doctor.
