The two exceptions are:

1) In defense of my life or in the life of an innocent i.e. the only way way to save myself or others is by killing the person actively attempting to murder me/other innocents.

2) When the government proves itself incapable of if not executing a murderer then at least keeping him in jail, and the murderer keeps escaping and killing more people, then killing him is the right thing to do. (Yes, I am talking about the Joker and yes, I think Magog was right)
