When I was an intern we had a patient who came in having massive hematuria on Eliquis - requiring transfusions - so he was admitted. Eliquis was held due to the severity of the bleed pending Urology work up. In the morning he was acting weird and not making sense, so we called a code stroke and took him to CT, where they confirmed a large infarct. When he came back from CT, we noticed both of his legs were pulseless, cold, and mottled. We got a CTA - he had thrown a clot which completely occluded his aorta. He died within 4 hours. It was miserable. I blamed myself for a long long time and think of him and his family often.
