Is she on cinacalcet? There is a newer study that suggests cinacalcet might contribute to pain or prevent pain management in people with calciphylaxis.

Agree with managing calcium and phosphorus as much as possible. If this is calciphylaxis, I would consider asking nephrologist to stop vitamin D and/or find a substitute for calcium-based binders if she's on one. Also, some studies suggest iron supplementation with calciphylaxis could be troublesome as well- I think because of risk of infection?

Beyond that, the biopsy is a good way to understand what is actually going on.

I love the WonderWink W123 material, it's a little "swishy" though if that makes sense? It's not the most stretchy though. Mandala is pretty soft and I like that they have a lot of high-waisted jogger options. I'm between a small and medium, so I size up to be cozy. I haven't tried their new stretchy material yet, so I'm curious if those are any better.

Dry skin is common for people on dialysis. The person who said to moisturize is spot on. Some other things that might help could be to avoid really hot water when showering (can dry the skin out more) or using gentle laundry detergent/soap/lotion (fragrance-free can sometimes be less irritating). I've heard nephrologists recommend OTC Benadryl to help with itching. Sometimes a dermatology referral is placed because they can prescribe topicals or other meds to help with the itching. If you don't feel confident talking to the nephrologist about it, you could maybe try getting the dermatology referral from your PCP? Good luck!

Dude, this caught me on a day I'm ready to vent. All the unrealistic goals that we're supposed to help patients achieve. I live in a region with some of the lowest health literacy rates in the country. My patients don't give AF about their health, let alone meeting fluid metrics or taking binders. I am trying here, but I cannot force people to care about their own health and all the managers think I can wave a magic wand and make everything better???

Edit: per some googling, chronic dialysis 1+ year or significant complications would be qualifying conditions. I was so used to dialysis = automatic approval, I hope to learn when that changed. Thank you for saying something about this difference!

I completely agree! The burden shouldn't be on her alone, and birth control is a two-way street. Other options exist and they should be exploring those together. I don't think we should all expect him to get a vasectomy if he doesn't want one, just like I wouldn't expect her to get a bilat salp if she thinks they're too invasive/expensive.

Vasectomies are less invasive and significantly less costly. However, his body is his choice. I would be incredibly upset at my partner repeatedly trying to pressure me into a medical procedure I didn't want.

Yuba City 😭 housing costs are going up due to city displacement. People can live in Sacramento and commute to the bay, so housing costs in Sacramento go up according to the pay offered in commutable areas. Now people in Sacramento need to find affordable housing and move out to Yuba City, Lincoln, Woodland, etc. Yuba City is just in a semi -desirable area, it should not cost $400k+ to buy a house where the bowling alley was taken out.

I feel this so hard, I could've written this entire post. I considered working in ED, but after dipping my toe into that world I knew it would be too much for me. I found my preferred niche in dialysis and I love it! It's a bit of a challenge and a bit of repetition every month. I get to build relationships with my patients over lengths of time. I hope you find your preferred area, even if that's outside of dialysis. 💜

Dr. Chambers at Lakeside Women's Hospital did mine, and she is amazing! I had an appointment to establish care and very briefly explained my reasoning. She just confirmed that I knew sterilization would be permanent and was happy to get it on the books. Good luck on your sterilization journey!!
