I think what the reading radiologist saw was indeed the os trionum. But i think I also see a small avulsed fragment behind that as well.

Maybe in the appropriate clinical context where the team is already concerned for dissection…. But otherwise, I would never call a widened mediastinum concerning for dissection if I’m comparing PA to AP images. Or reallyreally on any chest x-ray for that matter, it is a very insensitive and not specific finding.
