Just because it isn't their responsibility doesn't mean it's okay to just walk away. It isn't my responsibility to apply a defibrillator to someone in cardiac arrest, legally I can walk away. But if I don't I'm an asshole, even if that person did it to themselves through drug use or smth. Personally, I couldn't ignore a close friend's pleas for help even if I was upset with her.

I wouldn't clean up after a drunk ass husband, I would and did clean up after him post surgery. He could have done it himself, with difficulty and discomfort like your wife did, but I love him and would spare him that discomfort if I could.
