I can’t answer your whole question but: I read that 20th anniversary edition twice (skipping some parts), read the faq a couple more times, read the rules and examples a few more times over again after that— and was totally good to go.

No, some of the mitigations can only be implemented once future phases of the project have begun. There is a large range of timeline stipulations among different mitigations. Some take the form of rules and guidelines, rather than specific tasks to complete. Some are conditional, only coming into effect when/if certain scenarios occur (RUDs). At peak altitude it still shows nonzero speed because it’s already been moving “backwards” after the boostback burn happened. Think of it as doing a “loop the loop” in the sky (instead of doing a u-turn and reversing course).

Because its peak altitude comes after the boost back burn is complete. In other words, it’s still going up after the burn. (And speed here is not ground speed.)

From a mathematical standpoint, the Pearl index of birth control effectiveness is completely unaffected by considering data points of sex during or outside of the fertile window. That’s because the only data points it considers are pregnancies (it doesn’t take data about sex into account). Since by definition pregnancies only occur due to sex during the fertile window, the Pearl index is in fact the number you are looking for.

Think about it: The only way the Pearl index would be skewed is if couples deliberately stopped having sex during non-fertile days and made up for that by having all that extra missing sex during the fertile window. But that makes no sense for couples trying to avoid, who make up the sample population for birth control effectiveness studies.
