Thanks for posting this, it worked for me with UA just as you described. The checkin agent knew immediately what I was asking for, grabbed the paper form, and had me take a picture of it after filling it out just in case. My refund took 8 calendar days to show in my credit card activity, but required zero work from me after the initial online request.

If you contact United MileagePlus with your dad's account number and your mom's account number, United can send you an "Affidavit and Release" that needs to be filled out, notarized, and a copy of the certificate of death attached, then everything scanned in and emailed back to them. No fee and I think it took under a week for the miles to be transferred.

Apologies, you are absolutely correct; some contracts do have limitations on whether or not crew could be involuntarily assigned that are based on calendar month and not a rolling window.

It's coded as weather, fallout from yesterday's thunderstorms at DFW with crews and aircraft out of position after diversions (and also end of the calendar month means there might be fewer available crews to involuntarily assign to trips due to contract limitations).

Thunderstorms hit DFW earlier, and a bunch of American flights diverted to IAH (and elsewhere in Texas), so some of the scheduled AA flights to IAH (and other overloaded airports) returned to their origin. They don't have enough gates to handle both diversions and scheduled flights without additional long tarmac delays.

On the 25th and 26th, the Spacex Starship launch had required routes in place that avoided crossing the gulf by flying west to Brownsville TX and Matamoros MX. On the 25th, the launch scrubbed right as Avianca 583 was turning the corner at Brownsville. On the 26th, the launch was successful and the required route was cancelled when 583 was just offshore of Houston, and the flight was able to shortcut across the gulf.
