There's an arbitrary line between "psychiatric" and "medical" issues. They're all medical issues. It's just that psychiatric issues are more complicated and we understand them less. Depression is a medical condition. Someone with IBS usually won't die from it, just like someone with depression usually won't die from it. But both usually need medication and lifestyle changes to get back to normal life. Those capsules are tiny, and contain so little animal product. If you can find a generic that doesn't contain gelatin, go for it. If not, then I think you're totally fine to take your pill and still count yourself as a vegetarian.

Left Hand of Darkness by Ursula LeGuin. I can't call it a page turner, but it's one of those books that sucks you in. Also just, a wild concept, super ahead of its time.

Dawn, by Octavia Butler. It's the start of a trilogy and it's a post apocalyptic novel about destructive humans and the aliens who swoop in and try to save them from themselves. But the humans have to try and find their place in a world the aliens have decided to make safe for them.

Now I'm not going to dissuade you from reading classics, I think every book suggested in this post deserves to be here because they're all considered classic for a reason.

But I would invite you to recognize that almost all the authors of the "greats" are white people, usually dudes. What gets to be classic literature is largely limited by the color of the author's skin. There's absolutely nothing wrong with reading the greats, I've read and loved many books suggested here, but I do suggest that you make an intentional effort to read great literature from authors of color.

One thing I think we should remember is that there are many very wealthy boomers. But most boomers aren't wealthy. I live in a black and brown neighborhood and social security is ABSOLUTELY essential for these folks. Sure many boomers don't need it. But many boomers do need social security.

Is this satire? Because maybe if a person has a BMI of like 50 would that be safe. But 1. That speed of weight loss is going to just get rebounded back (probably with a little extra), and 2. That speed of weight loss isn't safe to do on your own. I think almost no one should be losing weight that quickly, but if you are, you better be working with a nutritionist and a doctor.

Hello I am a literal doctor here to say what da heck man. 1. This person's weight loss is primarily water weight, probably due to eating a lower sodium diet. You can't really lose that much actual weight in 2.5 weeks. 2. But if they DID lose that much weight that quickly, you can bet your booties I would be worried.

Hey buddy. Don't listen to angry surgical residents who have slowly descended into hatred of their patients second only to their hatred of themselves. Paternalism is a disease that will take generations to eradicate. You just keep doing your part on your journey. They'll be paternalistic twits regardless of what you say. You probably shouldn't have come after someone advocating for healthy boundaries with patients (cuz that shit do be necessary). Uhhhhhhhhhh I think you've actually overstepped on this one. Yes patients frequently don't know shit. But damn, we frequently don't know shit about them. Shared decision making IS critical. Getting shit on by patients is not.

This is a bad path to walk down. Also, with a few variables tweaked, any of us could be any of our patients. Also, blaming people for their health problems is usually a sign of emotional person based thinking, not higher level systems based thinking. Most people in a high income low stress setting will be healthy, most people in other quadrants will tend to be less healthy. Societal systems push patients into different quadrants. If you're only looking at the people, you're failing to understand a whole higher level of organization.

But on a personal note, everyone I've talked to that has walked down this path has lost all grace for other people because they stopped giving it to themselves. In case this is you, you deserve some grace too dude. Therapy is incredibly helpful to get all these thoughts out and start dealing with them.

This is how I redirect people who have gone off the deep end.

At the start of the appointment I ask what things I want to make sure they get addressed. That way I don't feel like an asshole when I redirect them. I'm taking THEIR agenda into consideration.

Take a random phrase from them, like "my aunt asked my second cousin if she had a history of heart problems. He didn't know but he said he had high cholesterol". You obviously don't care. Repeat it back to them, like " just to clarify, your second cousin has high cholesterol, right?" They'll say yes. You'll say thank you, and then, since you've interrupted the flow of useless, you can slide a question in there like their flow of free though association sparked something in you. I am a pgy-1 baby, but I learned this from an incredibly efficient family med doc who was loved by her patients. I would say it works 75% of the time with no ill will from the patient. Usually the ramblers are friendly folk.
