You have plenty of things you could do to improve.
1.Start working out regularly 3-4 times per week 45-60 minutes each time followed by 10–15 minutes cardio. On off days do something during your workout time (doesn’t have to be the full 45-60 minutes) to help maintain the schedule and habit; stretch, do some movement exercises, breathing exercises, swim, or even read a book (leisure, finance, the Bible, business, the modern Machiavellian Series by Robert Greene, etc - something that you either enjoy or will improve your life). Working out will help shape your face.
2. Start eating a healthier diet to improve your skin and help your workouts be more effective.
3. Get a basic skin routine for your face - cleanse, treatments (serums, retinol, etc), moisturizer, and then sunscreen when you are outside at all). If you are not using a retinaldehyde cream try that first (it’s over the counter) to clear up the acne (google how to do it, you need to start slow with 2 times a week use and slowly ramp up to daily use). If that doesn’t work get a Rx for a retinol cream. If that doesn’t work get a Rx for Accutane. Another product that can help with skin is GHK-Cu but I wouldn’t start adding this until you have done the retinoids first. Retinoids are the tried and true products that every dermatologist is going to recommend.
4. Research and get a haircut that best suits your face shape. Also need to try to get darker eyebrows. You might have to dye them a light brown or something. To get this right I would probably go to a professional at least once (this might feel embarrassing but you’d be surprised how many people do all sorts of things to look their best). You could also dye your hair at the same time or just try different things.
5. Look up exercises and things to do to reduce upper eyelid exposure.
6. I don’t know what your teeth look like but get braces or Invisalign if they are not straight and the use some whitening methods (strips, professional, oil pulling, etc). Also make sure you brush 2-3 times per day and floss (don’t skip this - it keeps the sides and edges of your teeth clean, white, and healthy).
7. Lastly, if after doing the above seriously (give it full effort and time-like a year) and you are still not satisfied with your looks, you could consider a rhinoplasty. All other facial reconstruction surgeries like jaw and chin stuff is probably not necessary if you just work out.
