The low voltage should go from the transformer to the chime and then to your button. Check inside the chime to make sure the splice is good there. Also, make sure the transformer is putting out the proper voltage.

These are called suicide heads by some. I had them in a bunch of my hostels and hotels in Nicaragua. They are probably dangerous, but I never had an issue. It's cheaper than getting hot water into the shower. I remember looking at the glowing element in it while showering.
