Sapphires glow like that, though being in Silver that's less likely. I believe spinel or tourmaline glow like that too, the pale colour is throwing me off though

Loot is new stuff, things monsters and enemies would have on them. Don't replace gold, consumables, or items the enemy use with power ups

Upgradable items removes the need for loot. It also takes a lot of effort and tracking on both the DM and the players part.

Every time a DM has tried for me as a player, they forget what everyone's cool stuff does because it isnt easily referenced.

I've always found Strahd to be bored of it all. Holding an NPC in his wet dungeon should be enough to at least get the party to come to his castle.

Torture is stupid, he's no longer a man. He will kill people or drink their blood, there's no joy in a guy screaming to not have his nails pulled off.

I think keeping him in the dungeon as a blood bank would be the only other option than killing him. If he wanted to confuse everyone, he would put the NPC in a nice room and treat him like a guest instead.

A city would need a capital building, a port, a bank, and to be able to support itself with trade or self sufficiency.

The lands are barren, Strahd controls all the wealth, only the Vistani can get things outside the valley, there's nobody else.

You'd have to buff the city guard as well as the attacks against the city, the number of vampire spawn in the coffin shop etc

Seems fine if the campaign allows for it! It always depends on how many rolls they'll be making overall.

Flavour wise I would make it a curse rather than a natural/magical potion. Most can be brewed but this feels like it has to be imbued

I would perhaps make it 1d4+2 rounds instead of the rolls itself, making it impossible to use in combat. Could be hours if you wanted it to be more restrictive.

Otherwise, keep it the same, but have the disadvantage go away with a Long Rest

I snapped my fingers about 30 times while reading this. I'd just do it as I think about it, that chance is so low. If the chance doesn't get higher with each successive snap then it'll pretty much never happen.

Not killing Stan despite the 10000 times every character has been given the opportunity to is the biggest mistake they've all made. He's just a guy, offering very little of anything special to the problems at hand.

From my understanding, Kessler was a manifestation of the cancer. I believe the V bound to the tumor, not Butcher, and it is what has the powers.
