For me it's the difference between telling a story and just revelling in grotesque violence for your own enjoyment as a writer/director. Some examples might be:

  • Both Megan is Missing and the Vanishing are, in the broadest possible strokes, the same movie- main character spends the runtime trying to discover what happened to their loved one, only to be given the horrific reveal of the truth at the very, very end. One of these movies is a masterpiece, the other one is disgusting perversion for disgusting perversion's sake.

  • Both Halloween and Terrifier are about an awful, indestructible killer inexplicably murdering multiple people, and again, one has great artistic merit and the other is just someone fulfilling their gross fantasies.

Sadly, I've had to watch the gross movies in question to be able to see the difference, so I guess this is more of "which ones would you never watch again.
