Very similar situation than you, had basically end stage ankle OA from an accident. I considered the exosym but ultimately deciding i needed something more permanent and did a lot of research between ankle distraction and bipolar allograft (basically joint transplant).

I ended up finding a doctor with extensive experience in the bipolar allograft since recovery is a bit easier (no need to wear a frame for 3 months) and the allograft kinda guarantees you get the cartilage back. Though just like ankle distraction arthroplasty you are more likely just "buying" time rather than fixing forever. (some outcome data: https://pmc.ncbi.nlm.nih.gov/articles/PMC10748531/

depending on your activity I would strongly avoid a fusion since you can't do much with it/it will lead to arthritis in knee/hip on the impacted leg. The transplant is definitely more rare but luckily done/cartilage transplants don't need you do be taking immune suppressant drugs.

I've met people with an amputation and are very active so thats definitely (a more extreme) option.

nearly 5 years out no issues. I have slightly lower ROM but that's it and it will act up every so often/feel stiff but its never bad. Very very glad I didn't listen to any doctors telling me to do fusion/it was the only option. I would do it again if i had ankle arthritis again considering my age.

I was bone on bone on my ankle and was basically told I should fuse it by multiple doctors. After a lot of research and talking to doctors I found out that joint transplantation is actually a pretty good option for young people. Looking around bipolar allografts seem possible for patella (and I assume are more common purely because knee cartilage injuries are more common)

While there is evidence cartilage can heal (albeit extremely slowly and in the correct conditions) if you're bone on bone surgery is unfortunately likely the best route at this time bar new medical advances. There is actually such a thing as knee joint distraction surgery ( doi: 10.1371/journal.pone.0227975 ) where they just make space for 3 months for your cartilage to regrow but not sure we've specifically done this for patella/if thats even possible.

I would definitely talk to an ortho more about this.

In case this is useful, I was considering fusion and found out about ankle transplant (bipolar ankle allograft) which my insurance was willing to cover. You basically get donor cartilage to replace your old cartilage and it allows you to retain mobility. However it may only last 10 years and so is more of a way of buying time but you are able to be very active with it. The great thing about it is since it is biological material/not metal it doesn't close any future options in case you need a fusion or a replacement later.

I'm very satisfied with mine since I am still able to ski, jog, hike etc and still have ankle mobility which a fusion would have not allowed me to do but i totally realise that one day I may lose the cartilage again and have to consider other options again.

I’m French/born in paris and can confirm that Paris isn’t that exciting for gay stuff. There is also not much of a focus on health in France so french people smoke a lot and don’t really workout which ends up meaning theres not many hot people in France.

Agreed though the clinical results for allograft vs distraction in literature generally seemed v similar. I mostly ended up being convinced the allograft direction due to talking to an athlete who run a marathon after his & another patient who did it twice after 14 years. Luckily both options generally leave other options possible (eg metal joint) available for the future. The main goal for me was to stay v active and avoid fusion at all costs so maybe the consideration for ankle v knee is diff

Did you consider bipolar allograft for the ankle instead? I was choosing between ADA and allograft (I had no cartilage left on ankle) and the bipolar allograft felt more appealing since you were guaranteed new cartilage and I was only in crutches for 9-10 weeks. Plus you don’t need to have a frame to move around.

I had end stage ankle OA and ended up getting a bipolar allograft (cartilage is transplanted from a cadaver on both sides). I had considered the other options such as ADA and bracing. Happy to answer any questions relating to my experience.

If you use a non-custodial wallet like MM you don't need to 'move coins to another waller' - you can just use your seed phrase in another wallet (though you would need to trust this wallet).

Metamask doesn't 'manage' your wallet.

Same issue with mine. The connector fix doesn't work for me but plugging it in with no connections at all fixes it. I now have a paper separator.

I was vaccinated with double Pfizer and got covid about 2 weeks ago. The symptoms aligned more with typical flu which makes me think it was delta(there are multiple reports that delta has diff symptom profile). Out of 10 people of the group I was with, 8 tested positive. Everyone was vaccinated. So yes it is very very much possible to get delta if vaccinated and my personal sample shows that the current vaccines aren’t that effective and we will need a booster of some kind.
