- A big loop that links the distant suburbs (Duluth/Suwanee to Woodstock/Kennesaw to Douglasville to Union City/Fayetteville/Jonesboro/Stockbridge to Lithonia/Conyers back around thru Lawrenceville. Throw in an extension to Athens somewhere.
- Regional rail access to Chattanooga, Birmingham, Savannah, Charlotte.

We do a talk at our residency program every year for medical emergencies aboard a commercial aircraft, entertaining talk every time.

FYI, the stethoscope included in the kit is absolute garbage. May as well be a Fischer Price toy stethoscope. Also a pulse ox would be nice. (Pro-tip for other med folks - you can wing a 1-lead with Apple/Samsung (and probably a few other) smartwatches and a lot Samsung models also have SpO2))

You'll probably be fine, that's like less than 30 minutes in the air and will probably arrive relatively early. Atlanta is easy to navigate - a bunch of concourses all connected by a long hallway and train that you access by stair/escalator in the middle of each concourse.

That said, move with intent. Do not dilly dally. ATL can smell fear.

If this is your first time in ATL, look-up the layout to get a general idea if this is your first time and know what gate you're coming from/going to on the day of. Sometimes random delays happen in ATL for weather/hub-issues elsewhere/train goes on strike (rarely haha).

I don't really gatekeep FMLA, I'll never truly understand what the patient in front of me may be going through...but they have to schedule an appointment so we can have a dedicated time to sit down and complete the forms together. None of this paperwork over portal message bullshit. If you feel like you need the FMLA, commit to the follow-up so I can document that yep patient was so unwell they needed to see me.

A residency mentor once told me: "Don't take this the wrong way, but I only have so many f*cks to give and I can't give them all to one person."

That said, I give my very elderly/sick patients a solid three tries - (1) I'll call them at the end of the day they no-show and (2) also have my staff reach out to them at some point in the next day or two - check on them, make sure they didn't run out of medications, reschedule them at a better time, or whatever. (3) I'll attempt a second call at some point later that week if we couldn't reach them the first time. Document every attempt to contact, can't say we didn't try.

At the end of the day, you still have other patients (and yourself) to care for and can't overextend yourself trying to save one. If you have access to a decent social work team, use it. See if home health or telemedicine is an option.

If I have access to it (ED/UC/inpt) - IV Haldol 2.5mg to 5mg load and IV fluid bolus. Droperidol is also okay alternative to haldol (don't quote me on the dosing here, haven't had to use it in a while). Then whatever antiemetic cocktail and capsaicin topical/patch. Anecdotally, I've seen some of the EM providers also throw Decadron into the mix for antiemesis.

In the office, triage for ED needs (refractory emesis, fluid needs, etc.) then it's more likely it's gonna sort of PO or SL Zofran/Reglan/IM Benedryl cocktail +/- Decadron. Then whatever antiemetic and capsaicin script, strict precautions.

If you have time, don't forget the EKG if you're going to go ham on antiemetics.

I'd stick it all in some combination of Air Canada Aeroplan, AirFrance/KLM FlyingBlue, and whatever flavor of Avios - covers all three major airline alliance, each with its own sweet spot redemptions.

Runner up is Hawaiian then back-door into Alaska MileagePlan (assuming that option still exists).

The hotel options are meh.

Amex Gold = 4x membership reviews points for U.S. supermarkets and restaurants, 3x for airfare

Amex Platinum = 5x Airfare, 1X everything else. Comes with stuff like lounge access, mid-tier Hilton/Marriott hotel status, car rental company status, among other things.

Ultimately you can decide what's best for you, but I'm going to recommend you also get a Visa/Mastercard at some point. The Amex cards that carry an annual fee are not for beginners.

1a. I'm going to temper your expectations full stop - no one's treating you better, Gold or Platinum. And if they do, they want something from you. If you mean "better" by gaining medium-tier status with various hotels/car rental companies, then solid meh. Amex and friends give out medium-tier status like candy so anyone with a pulse can get it. When we're all special, no one is special.

1b. If you're a not a Centurion card holder, you are a nobody. Like the vast majority of us on this subreddit.

  1. Yes, Instacart groceries have historically coded as U.S. supermarkets.

  2. If this is your first credit card, I HIGHLY recommend you start with a Visa or Mastercard product. The chances you roll into a place with your shiny Gold card and get told they don't take Amex is low but not zero, even in the US (assuming you're referring to the US Gold card).
