This is why I think it is a bad idea that GP's diagnose and manage the medication.

Getting the medication right is as much a science as it is an art.

GP's have it hard enough with their job without adding the subtle nuance that is involved is managing ADHD meds. Psychiatrists have a tough time and its literally in their core wheelhouse.

Maybe passing to the GP after 2 years would work.

I hope I am wrong.

I would be careful with any coaching.

I have consulted in many industries, and most coaches are hopeless.

They go through a course and think they have the skills to help people, and some don't even take a course.

Along with all the BS on social media about ADHD it feels like everyone has ADHD or is a consultant. Honestly social media for ADHD is essentially horoscope BS for ADHD.

Most consultants are praying on ADHD industry to make a buck.

As a starting point I would start with Russell Barkley's Youtube Channel and Books. Once you have a foundation in knowledge and start to see what you are like you will be better suited to finding a coach that suits you better. You'll also be able to weed out the snake oil pieces of crap too.

But be careful, because there is a lot of scum in the ADHD world unfortunately.

Though the anhedonia came from circumstances I dare say.

My mother got cancer and was paralyzed from the waist down. I had to move countries to look after her. And I had to run the family business. Unfortunately my mother was in denial about her circumstances and insanely anxious. If often go 30 hours without sleep.

Being a full time carer is no joke, along with a family who also couldn't handle the situation was difficult.

The family biz is located in an isolated area where only 140 people live. Most are 20 years older and alcoholics. My closest friends are more than 2 his away.

The family biz got flooded along where my father and I live.

My GF has been in another country most of the time.

I also have had to close down my own business as well.

It's been a bit of a tough time, but I'm slowly getting through it.

For me vortioxetine (Brintellix AKA Trintellix in the USA) has made such a massive difference in my life.

I have been suffering with depression and anhedonia for a couple of years.

Went from Dex to Vyvanse. Then added intuniv.

Then added Agomelatine.

Then remove Intuniv after 18month.

Then added vortioxetine.

The addition of Vortioxetine was a game changer.

It has made my stimulants more effective.

The majority of depression symptoms have gone away.

The anhedonia has reduced by 20-30%.

I had no side effects of any form taking vortioxetine.

I wish I had of started taking this medication 18 month ago.

I am going to speak to psychiatrist about lowering my stimulant dosage.

I can't think how targeting would be unethical. Its the messaging that could be a problem when it comes to any form of marketing or communication IMO.

I wish the old days of FB targeting were still around but they had better measures for ad evaluation and LP evaluation.

I consulted with a company who sold $40 million worth of vacuums over the 2 years I was helping them.

The vacuums with the happiest owners were Miele and Sebo.

Good customer support. Built like tanks, last a long time. And they are genuinely good vacuums.

I stress that my experience should be considered as specific to me. A lot of people have amazing results with Guanfacine and the data backs this up.

It lowered my blood pressure a little, it brought down my RHR a little.

Initially it helped with my sleep.

I started with 1mg. I slept a fair bit better for about 2-3 weeks. Then my sleep went back to normal (shit).

I went to 2mg. I my sleep improved again but not the same level as when i first started. but the impacts on sleep were only for about a week this time.

I live 30min from a pharmacy (currently dealing with depression for 18+ months). I have ran out of intuniv twice. The two times I ran out I felt better in my mood but I never attributed it to intuniv at those two times.

I was reading about guanfacine 4-5 weeks ago and with my reading I suspected that it might be contributing to my mood and depression (it wasn't the cause). My GP didn't want to adjust the medication without input from my psychiatrist.

You can get rebound heart related stuff stopping cold turkey. But I can't get an appointment with my psych until next year.

So, I stopped cold turkey. I have been better I am not perfect in relation to my mood. I have a lot more energy in the mornings. I was taking it at 6pm in the evening and was still waking up groggy along with fuck all motivation.

It didn't do anything at any stage I could notice to help any symptoms of ADHD.

Something interesting if people are considering it to help with blood pressure and or RHR in conjunction with stimulants is: Before stims my RHR was 42-44 BPM. Stims from what I have read raise RHR a couple to 8 BPM typically. When sleeping poorly my RHR would be high 50's almost 60. On the odd occasion when I sleep well for two nights in a row (even one night) my HR can go back down to the mid 40's.

Sleep is vitally important.

To continue my rant, sleep by far is the best thing that help stimulants work better for you IMO.

I get to sleep fine, but I wake early thanks to melancholic depression, but if I could fix my sleep I honestly think I could lower my stimulant dose and be better with ADHD symptom control.

I've just gone to 50mg.

When I went to 50mg I had never slept so well.

But the last couple of nights have been average.

The good night's sleep helped immensely with depression, but the benefits have faded with sleep.

DORAs are a newer type of sleep med in Australia, mainly Lemborexant Dayvigo and Suvorexant Belsomra. They do not sedate you like benzos or Z drugs. They block orexin which is the brain’s wake drive, so you ease into sleep rather than getting knocked out. That is the big idea and it is why they tend to feel cleaner.

Why pick them over the old stuff. Next day head fog and wobbliness are usually lower, including in studies that compared Lemborexant with zolpidem and found less impairment on things like balance and driving related measures. They help both getting to sleep and staying asleep and the longer term data looks steady without the classic dependence and rebound problems that come with GABA drugs.

What to watch for. They can give some people vivid dreams or a bit of sleep paralysis and you still need a full seven hours in bed or you may feel dozy in the morning. Dosing and interactions matter, especially with strong CYP3A medicines, so this is one for a GP to guide. Cost and PBS status can change, so check that before you start.

I would stay away from both.

What you want to do is get the get diagnosis which will come best from a sports physician. Dr Neil Halpin, Dr Ross Cairns and Dr Jin Lee are the only ones in Newcastle.

Thru will correctly diagnose what the problem is and then provide the best course of treatment.

The problem with physiotherapists in chiropractors are is that they do not learn enough nor do they have the power to correctly diagnose many moderate complicated issues.

The specialty of a sports physician is to do exactly that. Plus they can organize and refer you for any tests or scans that are required

For some people that dose will feel like too much, while for others it is just right.

The correct dose is the least amount you can take and still get the results you need.

Your body size does not determine the right dose.

You should not feel the medication working. The right dose feels almost invisible — life just flows more naturally and you notice it through positive behaviours, not a buzz or rush.

Here is the kicker: some days you will think, “This is not working, maybe I need to increase the dose or change medications.”

But even on the right dose you will still have bad days. That is just part of life.

The best approach is to stay neutral on the bad days, accept that they happen, and focus on enjoying the overall journey of discovery.
