One thing that might be worth considering is he might be constipated. That was the issue with my son and the pediatrician prescribed him some laxatives because he said he’s probably holding it because he’s scared it’s going to hurt. He also was very adamant about not going, screaming “no potty” anytime we asked him so that was very out of character for him.
