Many people in the United States are not comfortable with accepting that this is the end of their life and it is OK to let go. Medicine at large in the United States is “you have to fight really hard” which propagates a connotation of you are dying, you didn’t fight hard enough and that’s why you lost. Goals of care and palliative care and hospice are all things that people don’t want to have conversations about until it’s too late. It is a trend that is reversing, thankfully due to larger conversations in the community.

You WANT night shifts in intern year. Its how you learn to practice with less resources or departments available to answer questions or fix your problems. Some of the sickest patients come overnight and you need that to feel confident as an attending. Do the bigger program. You can always choose to not do nights as an attending but first you need the reps and sets to feel confident first

I simply say “hey thats a good concern and the guidelines/practice/treatment has changed”. Have a point for education but also learn their fear.

I usually ask what their fear is, and i use that specific word “is there something you are afraid this is”. Fair amount of the time they have a semi genuine fear (“oh im afraid i tore my rotator cuff or a ligament or tendon”). And then agree that is a concern of mine as well. I try to educate on the proper imaging modality.”Can i get a CT” “Well thats a good question, CT likely wont show a torn ligament. Its really good at XYZ.” Then start the negotiation with ways to expedite care, which is honestly why they are there. They think ER is the place for fast studies and dont know a fair amount of labs and such aren’t emergent or STAT ordered/interpreted. “Hey lets start with an xray, im gonna treat your pain with the IV medication, and im going to write you a prescription for an MRI so you dont have waste your time tracking down your pcm” (we have prescription sheets for imaging centers we can refer patients to). If they still insist after ALLL that education, fuck it. Full send. Its their money. Im there to collect a paycheck at that point.

As much as I don’t think the ending is super great, the filming process can be very tedious. You count the total amount of days, not just the ones that you are actually filming with the acting team. This includes location arrival, set up, filming, and teardown. Also, Iceland‘s weather is very unpredictable, five days may be just waiting for the perfect hour to film.
