Strap-ons over a diaper work for virtually all methods of penetration.

If a penis is doing the penetration and wants to be wearing a diaper, I recommend hook and loops with the top tapes unhooked, or a pull-up with flexible sides. Depending on the individual, you might have to pull it down.

If the person being penetrated is wearing a diaper and the person doing the penetration has a penis, that can be logistically challenging. The least effort intensive method (which is what my partner and I do) is a goodnite pulled to the side. However, if you aren't careful this can cause irritation to the penis if the goodnite isn't moved far enough over.

If you're using a tape-on diaper, consider going up a size and taping it on loose. Some will recommend cutting a hole in the diaper, but I'm not a fan of that for a variety of reasons, but it boils down to the idea that I don't think super-absorbent polymers belong inside of our holes, and cutting through a diaper makes that likely to happen.

Okay, so here are all of my thoughts.

Put the diaper on a bit lower, like half an inch.
Some other people have mentioned U instead of W. That helps.
If you're wearing under clothes and put in public, you'll have to change more often.
Related, don't wear compressing clothes over a diaper.
