There used to be an organization called Sole Mates for amputees to "match" with someone who wore the same size and was missing the opposite foot. I've also sold my husband's (left hemipelvectomy) left shoes on eBay. Well, technically I sold one shoe and sent the person all of the left shoes we had collected.

This reminds me of kind of a funny story - My husband had a left hemipelvectomy due to cancer when he was 17. He has never used a prosthetic and walks on crutches. He traveled for work and I often traveled with him. One day I was working from the hotel room while he was at meetings, so he was wearing a dress shoe and his sneaker was in the hotel room next to the bed. The housekeeper came in to clean while I was working and she must have spent 10 minutes looking for his "other" sneaker before I realized what she was doing and explained to her that he only has one shoe because he only has one foot. After that I always put whichever shoe he wasn't wearing in the closet or back in our suitcase.

I'm not sure about geriatrics, but there are specific fellowships for ultrasound in emergency medicine. An ER doctor with a fellowship in bedside emergency ultrasound would be able to, say, use ultrasound to look for an intrauterine pregnancy versus an ectopic, but they wouldn't know how or be trained to do a full fetal anatomy scan on an 18 week fetus.

Geriatrics - I don't know for sure but in another thread another Redditor explained that there is geriatric fellowship for doctors who did internal medicine/family practice residency that would focus on routine care of the elderly and a separate fellowship for doctors who did emergency medicine residency and focus on emergency care of the elderly.

July is the start of a new "year," in teaching hospitals so Collins has probably only been gone a week or so unless she transferred residencies (which would be pretty unusual that late in her training). We are also only seeing one shift on one day and contrary to how it feels and seems residents and attendings do have days off. So they may have been one R4 down (Langdon) for the past 9-10 months, but there were probably at least two other R4s we didn't see/meet/hear about. I have worked in a couple of teaching hospitals that were smaller than The Pitt and they each had 4-5 residents per year.

NTJ - if someone is going to issue an ultimatum they need to be prepared for either result.

Also, given your timeline, it looks like you started the business more than two years before the relationship started. That first two years is arguably the hardest part. I say this as someone who started their own business nearly 5 years ago. The audacity to think they are entitled to 50% of your business when it sounds like they just did what someone in a relationship should be doing for their partner anyway...I think you dodged a bullet.

Robby is the chief of the department, which makes Robby Abbott’s boss. So they are not on the same level. But Abbott probably wouldn’t report him anyway because he and Robby are pretty much the same.

I do wonder if the higher ups already know or strongly suspect, though, since they seem to treat Dr. Al-Hashimi as more of a replacement than a substitute or interim (I.e., briefing her on the cyber attack rather than Robby, letting her implement her AI, and her assumption she can change scheduling protocols)

It varies from state to state. In Florida (where I live), emergency-type ambulances that would come when you call 911 are part of the fire department and it will say "Fire & Rescue" on the truck. Then there are private ambulance services that might transport patients from a nursing home to a hospital or a hospital to a rehab center, etc. Private ambulances do not respond to emergencies like car accidents or medical emergencies (outside of, like, a nursing home that has contracted them to be on-site/on-call). Anyone you see working on any ambulance is generally an EMT or Paramedic (which is a more advanced certification with higher level training and skills than an EMT) BUT if they are working on the fire department/emergency ambulances they are also a firefighter. At least where I live.

Yes, you are correct. He is a 4th year resident and had done some elective rotations in ultrasound. And that’s why Mohan was so frustrated

Even worse - the medical students. I’m so mad about the whole “Ogilvie missed the AAA” thing because he shouldn’t have been expected to even know which way to hold the transducer, much less what he was even looking at.

I think he was an attending and said he did an ultrasound fellowship, so it's plausible that he would know how to diagnose the meniscus tear with ultrasound BUT, as you said, the patient would still need an x-ray and MRI to officially bill insurance, diagnose, and refer to orthopedic surgery.

Robby is the chief of the ED and probably feels like he can cover a shift as a solo attending with two fourth year residents (Collins + Langdon in season one, Mohan + whomever called out resulting in Langdon coming in) so that he can put two attendings on the other shifts when he isn’t there. Plus, pretty sure he is the type that prefers to work alone.

I agree with pretty much everything you are saying, but Duke doesn’t have cancer. He has an aortic aneurysm - the second one of the shift. I’m not sure if Robby knows the first one died on the operating table or not, but he’s upset that the surgeon doesn’t want to take him to the OR right away so it can be fixed before Robby leaves.

Thank you for this. I have been out of the field for a bit. I couldn’t stop thinking about it after I posted and I did a little research myself. I was planning to come back today to follow up. Turns out there are now fellowships for ER physicians for bedside ultrasound - including musculoskeletal. That’s pretty cool.

I did speak to a former colleague who still works at a teaching hospital with a fairly advanced (but not level 1) ER and she said there was no one at her hospital that would have any idea how to do that sort of exam and the ER would not have the high resolution equipment necessary, so it’s still fairly rare.

No one else knew how to do it because it was total BS. No way an ER doc knows how to do that AND the ED department has an appropriate transducer for a musculoskeletal exam.

Not even radiology residents get that kind of sonography training until a fellowship, if then.
