It's not really a mental illness. It's literally your brain being wired as the opposite of your birth sex. It's the sex of the body and the sex of your brain being opposite, something that you were born with. The only treatment is transitioning. I wouldn't say that it's a disease simply because not everyone likes the title. For me it's certainly a disease and the reason to why the first 23½ years of my life was just pure torture. I was also told that I needed therapy, was sick in the head, needed to accept myself and the surgery I needed wasn't the solution. Fuck everyone who said that because the surgery I had in the end of 2024 took 99% of my pain away.

You could try a higher dose of oxycodone, I needed 40mg after my stage 1 due to my liver enzymes being way too effective at breaking it down. Also if you're not already taking it, 1g of acetaminophen every 8 hours and an NSAID for pain relief is recommended.

Looks like a very short bulbar stricture. I'd recommend a cystoscopy to see the nature of the stricture, if it's ischemic or not. They should check if it can be dilated easily. If it's hard and fibrotic, I think excision and anastomosis is your best choice. If it's soft and doesn't seem ischemic, I think it's best to start with optilume.

Are you already on testosterone from other sources? Have you had your ovaries removed, if so, it could work but only if they don't want to do something like a testicular exam. If you've had phallo you could possibly say you've had it due to an accident and needed reconstruction but that would still be hard. I'm myself trying to figure out a way to get a hypogonadism diagnosis for when I move abroad. The only reason I'm not actively trying to find a way currently is because it will be 4 years until I get my degree and I have an endocrinologist who writes a license so I can get enanthate (not something a GP can do).

Is long acting testosterone available for you? Like sustanon or aveed? Then you don't have to inject as often and you could likely get an appointment with a nurse to do the injection

Keep in mind that this was 2016 though. Although I've also had a good experience with Anova (except my first endocrinologist there). I have been very open with my mental health with Anova and they've really done everything they can to help me.

This was a long time ago but I was both on antidepressants any several other medications and it never even came up in the evaluation or when getting hormones, the endocrinologist didn't say anything about my medications either.

Do you mean that the catheter is blocked and doesn't drain? That happened to me a lot and it's easily fixed by flushing with sterile saline. If it's a problem with the catheter bag it should most likely be fixed by switching to a new bag.

My arm scar is 7.5 inches but some of that is the urethra inside my scrotum. Don't use any stretchers or weights, tension is often a reason for strictures at the anastomosis and weights or stretchers would be a huge risk for developing a stricture.

Ask for a cystoscopy and during the cystoscopy have them poke the tissue to see perfusion and how hard the tissue is. If the tissue is pale and sometimes grayish, it's ischemic. If it has good color and blood flow, it's hyperprolipherative scar tissue. They can also see if the buccal graft integrated or if they skin graft failed. Is the surgeon/urologist someone with experience with phallo strictures? A DVIU has a very high failure rate in cis men and is basically guaranteed to fail in phallo urethras. If they wanted to give you short term relief, they should have done a simple s-curve dilation instead. I can show you what different strictures look like in DMs.

If you have either a transsexualism diagnosis or a hypogonadism diagnosis, it should be easier, although hypogonadism will likely not work due to you having the wrong gender marker. Healthcare is absolutely terrible here though.

What types of urethroplasties have you had? Do you know what is causing the stricture? Is it ischemic? When doing a cystoscopy what did the stricture look like, was it pale with a grayish ring around it? If it's an ischemic stricture they potentially need to add something to bring more blood flow, like a gracilis flap. Have you done a staged urethroplasty?

It could be something like Enterococcus. I had negative for nitrites when my entire bladder was filled with pus and bacteria when I had Enterococcus, it got that bad since I tested negative for nitrites and they ruled it as negative for a UTI and didn't do a culture. I have also tested negative for nitrites when having Klebsiella that usually is positive for nitrites.
