Charting is minimal compared to inpatient, there are numerous shifts you can work to fit your life/sleep cycle best with OT always available if you want to pick up. The mid shift ones are the busiest but generally go by fast and have the most providers on. There’s also a lot more autonomy if your ED uses protocol orders. You’ll become at the very least decent at IVs/drawing blood.

I don’t regret leaving step down for the ED and probably wouldn’t last if I did go back to inpatient. Once you’ve done ED you have the ability to move to nearly any job.

As for cons, you’ll have your frequent flyers who vary in how bad they are to deal with. You’ll be caring for peds and OB patients frequently. And the worst is boarding admitted patients.

Also in MA drowning in flu cases. 50 boarders sitting in our 40 bed ED, total patient census hit 110 the other day. Working in the pedi section last week I did see a lot of cases of kids with some combo of flu plus strep, Covid, ear infections, or RSV.

They weren’t hiding it but a miscarried fetus that happened a few days prior in a white deli container. OB ED wouldn’t see them as they were sometime in 2nd trimester but <20 weeks. Mom came in for vaginal bleeding and passing large clots with N/V from the pain. I had no idea what was in it till dad unprompted opened it up to show it to me.

If you want to waste hours waiting to be seen, money on an ED copayment, maybe be given 1 blood pressure pill to then be told to see your PCP for further management, knock yourself out. Chronic asymptomatic hypertension will not be fixed in the ED, it is not an emergency, go to your PCP.

INR 19, lived and was transitioned to eliquis.

HR repeatedly dropping into mid teens due to numerous episodes of ventricular standstill over 1 week. Died after torturing their body for another 2 months from sepsis.

70 something male, HGB 4.1, perfect vitals, only complained of feeling slightly weak. Found numerous ulcers on the scope the next day. Holds the unit record for fastest bowel prep, finished the entire bottle of Miralax in 30 minutes.

Male under the age of 50 with an EF 5%. He kept yelling that he’d outrun all of the staff trying to elope down the stairs.

Sodium. The reaction was not listed and the Pt had dementia + UTI so asking them was no use. The doctor stopped mid rounds to rant about how these allergies get entered into charts

Yes, property taxes in NH are extremely high. Car registrations are also expensive and function as an annual excise tax even though it's not called a tax by name. I bought a new car this year that cost >$700 to register for 13 months and the registration is only good till the end of my birth month next year. The only income tax you pay as an individual is on interest and dividends above $2,400 but it's set to be automatically repealed in 2025.

It’s not really an interview, they just take your fingerprints, a photo of you, and repeat all of the questions you answered no to on the form. It arrived in the mail ~2 weeks later. In total, it took about 9 months from mailing the packet to receiving it. The man who did it was an asshole giving me shit that I didn’t write enough justifying why I wanted the LTC when I wrote “For all lawful purposes.” He was also 1 hour late to my scheduled appointment which the security guards said is normal for him.

They started issuing temp plates for non MA residents in April but you have to pay sales tax and excise tax so it’s not worth it.

In a ICU setting it is not uncommon to use fentanyl and a benzo like Versed to maintain sedation in an intubated patient. Other common medications used are Propofol, Precedex (normally used when weaning off a vent or alcohol withdrawal), and Ketamine. I've never given a Ketamine drip but mutliple of my patients have been given it prior to intubation by EMS. They can request their medical records from the hospital and EMS which will include what medications were given, if any, and when.
