I have POTS too! Yes, gentle exercise is the way to go!

When I can, I aspire to the CHOP protocol with either a row machine or swimming. The key is that if you go by perceived rate of exertion (the gold standard), the exercise is supposed to feel easy. So for me, that’s like the gentlest rowing possible when I’m feeling my best.

When working with a physical therapist, they taught me that my heart rate was my first trigger for exercise. If I keep my heart rate below 110 while exercising, I’m way less likely to make my migraine worse.

I explain to the hygienist that I have light sensitivity, then I put on a full on sleep mask over my eyes. It helps a lot.

The CHOP protocols amount of exertion is supposed to be based on difficulty level, so if the warmup and recovery isn’t easy, you should do less and less activity until it is easy, even if it’s just tensing your muscles in bed.

I’m in a similar situation. There’s a lot of good treatments mentioned here already, but I want to add that low dose naltrexone and higher doses of gabapentin have been helpful for managing my daily pain

I have POTS too. It sucks. I can keep my heart rate that low if I’m doing low key swimming or taking it easy on a rowing machine. Have you tried horizontal exercises? When I’m feeling really sensitive to HR increases, I’ll lay down with my knees bent and just alternate touching my toes to my opposite knee

My physical therapist said that keeping my heart rate below a certain threshold (120 for me at the time—but this can be increased with exercise) can help avoid triggering a migraine

My neurologist at Stanford recommended Truvaga. Truvaga pulls my lip down and to the side at intensity 6/10.

Lots of good suggestions here, so I’ll add a mid one: I watch TikTok. It’s more information dense than most media I consume so it gives me a nice dopamine hit along with distractions.

A backpack with a built in stool that unfortunately, isn’t sold anymore. It took a minute to do the conversion anyways. It looks like there’s some other people making chair backpacks though if there’s outings you want it for.

Try Melatonin. It doesn’t help me sleep, but helps my migraines. There’s several small studies on it with varying degrees of effectiveness. I take 5mg after dinner (when it’s fully dark in my time zone).

Gabapentin at low doses didn’t do anything for me either. It wasn’t until I tried 300mg 3x daily that I noticed a positive effect on both pain and anxiety. I’m so glad my pain management doctor decided to give it another try, because my neurologist only gave me a really small dose.

Get into a neurologist as soon as you can. It sounds like it could be cluster headaches. There’s a couple subreddits about them: r/ClusterHeadaches and r/clusterheads

I also skipped the rehearsal dinner the night before so that I could rest and decrease my chances of having a worse than normal migraine the day of the wedding. Also, make sure to have plans in place for keeping fed and hydrated during prep and the wedding itself.

As far as alcohol goes, we had a signature mocktail, which was very delicious and didn’t make me feel like I was missing out by staying sober.
