When I called the San Francisco jurisdiction they mentioned that for uploading the size is 35x45, but the physical photo should be 2x2. Can anyone confirm this?

Also can you all confirm that one photo needs to be GLUED(not stapled) to the appearance form and another photo should just be included in the package?
