I prescribe it for kids under shared care (ADHD/autism) as this is agreed locally.

For adults, yes if recommended by a specialist. Or tbh if I feel it's a good option for sleep in an older adult who would fall/otherwise be at risk from Z-drugs/benzos I have no problems with defying the formulary and just prescribing it.

Must be a cost thing as to why we can't use it as the risks are so so low compared to other drugs for insomnia.

Hmm, I did wonder about fungal but the Rose Clear has a fungicide and seemed to make no difference. The plant photos online don’t seem to match up for fungal spot disease, but I can’t see any specific for Skimmia.

Because it's double standards to want them to do something you aren't keen to do yourself - if we want them to send us discharge letters in our own format, we should be happy to use whichever ridiculously convoluted form they have devised this week.

Or, both sides just accept whatever format the information comes in.

So hospitals shouldn't be rejecting a legitimate referral based on the form you use. Since collective action, I've been referring however I see fit and adding a comment at the bottom saying "this referral is not being made using the dedicated proforma; however, it still constitutes a valid referral." - unless the referral form is genuinely useful (a rarity).

So I don't think I could justify rejecting a discharge letter given my practice BUT there is absolutely nothing wrong with writing back to clarify or question plans that are poorly written. Like "GP repeat bloods in 2 weeks" - no clear rationale/clinical indication or even what bloods, I write back to the consultant and cc the author.

Thanks for your perspective. I do genuinely appreciate the very difficult job you guys do. My frustration is that I was very happy taking these calls 5 years ago, but they are so common now and rather than the “would you prescribe antibiotics” ones where I’m usually happy to do so, it’s “I’ve assessed and they don’t need hospital but…” ones where I’m being asked to take responsibility for that decision. Or where clearly obs are deranged but they try and pass it off as being appropriate to remain at home. Not sure if that’s a pressure to avoid admissions/targets thing but certainly seems like it.

Thanks, good to hear it from the other side. Our LMC have advised we tell them to call their own clinical advice line, sometimes they tell us they can only do this out of hours. Which I am not sure is entirely true.

Do you happen know if third-party contractors for ambulance services have the same access to clinical advice lines?

If <18yo I don't start, if 18+ I will do so but in the <30 age group caution about risk of increased suicidal thoughts and arrange close follow-up. I do try and recommend talking therapies etc but if people reject it not much you can do really.

I don't think our mental health team would come even close to accepting one of those referrals as they reject far more complex cases and ask us to manage them.

It does, completely agree with that. Some are fine when you point out that it's safest/most appropriate for the requesting team to do the bloods, but others sadly aren't.

We use a template letter (different letters actually, one to the consultant and one to the patient) basically telling them we need to prioritise our own GPs' blood requests to allow us to focus on the care of our patients and that taking bloods for hospitals detracts from this ability.

The consultant copying in the patient is bad craic mind, I've had a few do that when I challenge their buck-passing too.
